**ABOUT THE OPPORTUNITY**:
The Caring Connections Home supervisor works in partnership with the YCPs, other professionals, youth and families in developing an overall healing process within the program and for individual youth. The Caring Connections Home Supervisor is responsible for the daily operation of the home, the case management of all youth within the program and all personnel matters related to the staff team. The Caring Connections Home supervisor functions within a strengths based, trauma informed, Relational Child & Youth Care approach.

**PRIMARY RESPONSIBILITIES**: of the Placement Stabilization Unit Supervisor include, but are not limited to:
**Treatment Planning and Implementation**:

- Develops, with the YCPs and other professionals, placing agency social worker, youth, and family, specific goals and / or assessment planning for each youth in order to return to their placement.
- Supports and ensures, through supervision, that plans are effectively implemented by the Youth Care Practitioners / team;
- Meets regularly with other involved professionals for the purposes of collaboration, planning and updating the return to placement plan;
- Ensures that all activities, routines and interactions are therapeutic and philosophically congruent with the program;
- Provides supervision to the Youth Care Practitioners to ensure their actions / interactions with youth and families are consistent with the planning for the youth to return to their placement;
- Ensures that the YCPs are interacting with youth and families from a strength-based, trauma-informed, developmental and relational, Child & Youth Care approach.

**Case Management**:

- Is responsible to oversee all aspects of the interventions described for all youth, ensuring other professionals, youth’s, families and others input into clinical aspects of planning for each young person and family;
- Ensures that the referral, intake and discharge processes are conducted in accordance with Provincial and Organizational standards and procedures;
- Ensures that in addition to Intake meetings, system meetings and planning meetings are scheduled for all youth while admitted to the Caring Connections Home;
- Ensures that all documentation is clear, accurate, and professional and complies with Licensing, Program standards and The Link policies and procedures;
- Coordinates communication with the Clinical Follow-Up Facilitators, external collaterals, and families and with other Link programs and resources as needed;
- Participates in hiring, supervises and evaluates all Youth Care Practitioners under their responsibility, in accordance with organizational standards;
- Maintains staffing to meet youth, family and program needs;
- Provides support and guidance within the program as it relates to the youth and to the staff team;
- Ensures that the Youth Care Practitioners understand their job expectations and provides ongoing support to meet the expectations;
- Provides mentorship and training / education to the staff in helping them to develop the skills and knowledge necessary to work effectively with the youth;
**Unit Management**:

- Creates and maintains, through the supervision of YCPs, a therapeutic and home like environment within the program;
- Ensures the physical maintenance of the home and its contents and property as well as all equipment and vehicles of the program;
- Ensures compliance with Residential Licensing, provincial program standards and The Link standards;
- Contributes to the development of the program budget and ensures all decisions pertaining to purchasing are within that budget;
- Ensures that the scheduling of youth care staff meets the youth and program needs;
- Schedules and chairs biweekly staff team meetings focusing on unit functioning, team functioning and individual youth planning;
- Develops and maintains systems to ensure effective internal communication and documentation;
- Implements an on call system that is reflective of program needs.

**Continuity of Care**:

- Works in consultation with other Healing Homes Supervisors, CCMs, Therapists and the Manager of Operations to ensure
