Join us in making a meaningful impact **Summary of Position** Reporting to the **Director, Quality and Performance** and the **Community Wellness Hub Coordinator - 8 week contract**, you will have the opportunity to learn and develop your skills, and knowledge while gaining valuable experience in the Community Wellness Hub model. The Community Wellness Hub, as a partnership model, exists to provide a variety of health and social care opportunities directly to residents where they live. During your placement, you will complete several programming, administrative, and client interaction tasks and projects, and work with internal and external key partners in delivering programs and services to our clients. **Role and Responsibilities** - Support and work collaboratively with Community Wellness Hub Coordinator for development and implementation of events and other programming. - Support clients directly when at events and programs. - Participate in assessments, surveys and interviews of clients and complete appropriate paperwork accordingly. - Support in administrative tasks tied to client support and referrals. - Use excellent customer service skills and find solutions when working with clients. - Complete data entry and data maintenance. - Photograph clients with their consent at community or in-house events that occur as part of the wellness hub. - Support in dealing with client concerns or conflicts and work alongside Coordinator to support meaningful outcomes. - Use troubleshooting, problem solving, and de escalation techniques while working with residents in crisis. - Report back regularly to Community Wellness Hub Coordinator and/or Community Wellness Hub team for any questions or concerns or to debrief and discuss. - Ability to lift 50 lbs., i.e., setting up tables and chairs for event set up. - Maintain workload and be able to attend meetings as required. **Summary of Required Skills** - Completed or enrolled in a post-secondary diploma or degree program. - Interested in learning and developing knowledge and skills valuable in in community and health services, gerontology, persons with disabilities, and/or support health services administration. - Experience in programming from implementation to evaluation with a focus on socialization and recreation. - Enthusiasm to work with those who are 65 years and older or those living with a disability. - Have empathy and compassion and understanding of working with vulnerable clients. - Excellent written, oral and telephone communication skills and ability to communicate effectively when unsure of a task. - Effective troubleshooting, problem-solving and de-escalation techniques and skills used when dealing directly with individuals. - Basic knowledge and experience with MS Office Suite, including Excel. - Organized, detail oriented, and collaborative. - Likes to take initiative and be a self starter to make improvements and complete projects. - Ability to collaborate and receive feedback in a professional manner. - Takes pride in completing work without errors and in a timely manner. - Completion of a satisfactory Criminal Record and Judicial Matters Check (CRJMC) prior to and throughout employment as requested by Links2Care is required. - Employees who drive on behalf of Links2Care for work related purposes must possess and provide proof of a valid Ontario driver’s license and a minimum of $1,000,000 liability insurance on their vehicle. **Job Types**: Full-time, Fixed term contract, Seasonal, Internship / Co-op Contract length: 8 weeks Pay: From $17.20 per hour **Benefits**: - Company events Flexible language requirement: - French not required Schedule: - Day shift - Monday to Friday Work Location: In person
