**_
KEY RESPONSIBILITIES_**_:_
- _ Receive and respond appropriately to inquiries pertaining to Southbridge Roseview which relate to new resident placement, including conducting tours of the home with prospective residents;_
- _ To promote and facilitate a Resident-oriented environment, acting as a Resident/family advocate. Coordinates and is the homes liaison person for Family Council. Leads the Customer Service program within the home;_
- _ Coordinate, plan and participate in annual and new admission interdisciplinary team care conferences for residents;_
- _ To plan, organize and carry out social work programs/services to meet the ongoing psychosocial needs of the Residents and their families/advocates; _
- _Provides palliative care support to residents and family as needed;_
- _ Examines and acts on matters concerning possible financial abuse or emotional abuse of a resident and works collaboratively with interdisciplinary team to address the issue;_
- _ Leads the ethics committee for the home and is available to staff to consult with regarding ethical dilemmas within the home, and works collaboratively with staff to resolve ethical dilemmas; _
- _Participates on committees related to resident care and operation of the facility as required;_
- _ To function as a member of the multi-disciplinary team in planning overall Resident care;_
- _ To provide educational and consultative services on social work related issues/topics through participation in staff orientation programs, staff development programs and educational seminars for staff, Residents and families/advocates as required._

**_ KEY QUALIFICATIONS_**_:_
- _ Registered under the Social Work & Social Service Act, 1998; Graduate of a recognized school of social work with a Bachelor of Social Work degree; _
- _Experience in working with Geriatrics; Familiarity with the multi-disciplinary team concept, skill in teamwork and ability to work effectively in a multi-disciplinary setting; _
- _Minimum of 3 years' experience as a Social Worker in a clinical setting; _
- _Strong knowledge base for mental health issues;_
- _ Excellent team player with strong interpersonal and communication skills; _
- _Ability to respond to crisis situations with demonstrated critical thinking; _
- _Demonstrated ability to function as a team player;_
- _ Good communication and interpersonal skills; _
- _French Language skills will be considered an asset;_
