**Job Summary**

The Rural Outreach Support Worker is responsible for offering trauma-Informed, wrap-around supports to individuals who have experienced abuse and are living in the County of Brant or in transition from the shelter to the County of Brant. The Rural Outreach Support Worker is responsible for managing a case load and creating individualized transitional plans which include, but are not limited to: safety planning, risk assessment, finding and maintaining housing, individual and group counselling, employment/income supports, parenting support, referrals to internal programs and to community organizations, legal/court support, accompaniments, system navigation, and advocacy. The Rural Outreach Support Worker is also responsible for engaging in community education and raising awareness about domestic violence, intimate partner violence, and gender-based violence in the County of Brant.

**Qualifications**
- SSW, BSW or an equivalent diploma/degree in a related field with experience in the Social Services sector is required
- Member of Ontario College of Social Workers and Social Service Workers or an equivalent regulatory body is preferred
- Experience working from a trauma informed approach is an asset
- 3 years’ experience in the anti-violence sector or related social services sector is preferred
- Must possess a valid driver’s license and access to own insured vehicle
- Police check with vulnerable sector screening required
- Crisis Prevention/Intervention Training is required
- Valid First Aid/CPR is required
- Mental Health First Aid and/or ASIST Training is preferred
- Verification of COVID-19 vaccination is required

**Essential Knowledge, Skills, and Abilities**
- Demonstrated experience working with diverse populations from an intersectional, inclusive and anti-oppressive lens
- Excellent understanding of the dynamics and impact of intimate partner violence, domestic violence, family violence, sexual violence, childhood trauma, human trafficking and/or gender-based violence
- Experience in safety planning, risk assessment, crisis intervention, counselling, and providing appropriate referrals to individuals who have experienced violence, abuse, and/or trauma
- Excellent knowledge of Nova Vita’s programs and services
- Excellent knowledge of community resources in Brant County and surrounding areas
- Experience in providing individual crisis and supportive counselling and identifying appropriate client-centred referrals
- Experience in facilitating group programming is an asset
- Ability to manage competing demands in a fast-paced environment while balancing a caseload;
- Ability to take initiative and work collaboratively in a team environment
- Ability to work independently in a community setting with mínimal supervision
- Ability to be an ambassador of Nova Vita, liaise, and work with community-based organizations
- Excellent communication skills, both written and verbal
- Excellent interpersonal, professional and time management skills
- Computer literacy and proficiency with Microsoft Office and Zoom

**Duties and Responsibilities**
- Provide emotional and practical support to women, women-identified, and gender diverse individuals who have experienced (or at risk of) violence, abuse, or trauma and
- are in transition (from community to shelter or shelter to community), or
- need support in maintaining safe, affordable housing, or
- need support in establishing a life free of violence
- Create individualized goals, transitional support plans, and safety plans with each client
- Communicate and collaborate with external community agencies to support client goals and make appropriate referrals
- Assist clients with system navigation and advocacy to reduce barriers in accessing services
- Provide accompaniments for clients to appointments when required (housing, medical, court, lawyers, etc.)
- Organize case conferences with community agencies as needed
- Maintain a flexible work schedule when required, including evenings and weekends
- Maintain accurate and timely documentation
- Liaise with Brant County organizations and facilitate community education and awareness as needed
- Assume other duties and responsibilities as required

**Accountabilities**
- Adhere to relevant legislation and agency-wide policies & procedures at all times
- Adhere to a strict policy of confidentiality
- Maintain health and safety requirements at all times, including COVID-19 prevention protocols
- Maintain professional and ethical boundaries with clients and community partners at all times
- Complete all documentation as required and in a timely manner
- Engage in regular supervision with the Community Counselling and Outreach Manager

**Job Types**: Full-time, Fixed term contract
Contract length: 10 months

Pay: $26.06 per hour

Expected hours: 37.5 per week
