Sagkeeng Child and Family Services (SCFS) is a First Nations CFS Agency who strives to recruit inspired, accountable, and driven individuals to join our diverse talented team of professionals. We are currently recruiting for a Prevention Service Facilitator for our Agency.
**Job Summary and Qualifications**
**Areas of specialty**:
This position is responsible for providing assistance in identifying, enhancing and developing programs and services that meet the needs of people from the Community of Sagkeeng First Nation. The Facilitator will be responsible providing one on one support with children/youth and families and assist the family to preserve the family unit by assisting in identifying their strengths to complete the agreed upon case plan.
**Qualifications**:
- Post-secondary Degree in Social Sciences; combination of education and experience may be considered
- Experience within a child & family service setting
- Knowledge of the Wraparound Model is preferred
- Knowledge and appreciation of Sagkeeng’s customs and traditions
- Proven experience with documentation and writing
- Strong knowledge of community resources and ability to maintain professional relationships with families, colleagues and collaterals
- Flexible work schedule (able to work evenings, weekends, possibly holidays).
- Ability to work independently as well as part of a team.
- Able to lift 75-100lbs.
- The ability to speak and/or understand the Ojibway language is considered an asset.
- Valid class five (5) driver’s license and fully insured reliable vehicle with $2,000,000.00 liability insurance
**Duties & Responsibilities (include but are not limited to)**:
- Assists with program delivery, including engagement, preparation, implementation and transition.
- Build relationships with the families in order to assist the during their healing journey
- Build a supportive team with the families
- Invites all team members to participate in the Circle of Care meetings
- Leads the Circle of Care meetings and creates a mission statement, Cultural Needs, Vision and Discovery. Builds a Circle of care case plan with the participant and supports.
- Build a wellness plan to prevent participant from crisis and to prepare participant in case the crisis occurs.
- Motivates and encourages participant to work on themselves/case plan
- Provides holistic support to participants
- Works along with the case managers assigned to the families
- Transitions the family out of the program
- Assist with the Celebration when the family completed the program
- Connects the families to internal and external resources
- Performs other duties as assigned.

**Salary**: $48,285.00-$59,557.00 per year
