**The Position**

Reporting to the Education Division Manager and the supervision of the K-12 Education Manager, the SCD Case Manager works in collaboration with parents/caregivers, early childhood education staff, school staff, other Nation teams, and relevant service providers/agencies. You will assist in developing, implementing and evaluating individual service plans for shíshálh children from birth - 12 years of age who need support in any of the following areas: physical, cognitive, communication, social-emotional and/or behavioural development.

General Responsibilities
- Facilitate the development and implementation of individual service plans for children with additional support needs, ensuring plans are child-specific, family-centred, culturally affirming and developmentally appropriate and that they reflect the choices and priorities identified by the child’s family.
- Respond to the child and family’s holistic needs - making referrals to other departments and community service agencies as requested and appropriate.
- Provide one-to-one support within schools and the Nation's early childhood and out-of-school care programs on an as-needed basis to ensure inclusion; assume the role of one-to-one support worker during summer months.
- Provide supportive professional relationships with the parents, caregivers, and extended family of the children on the caseload.
- Perform other duties within the scope of the position, as assigned.

Case Management
- Work within an integrated team approach with professionals for speech and language, occupational therapists, consultants, teachers and school-based staff and/or childcare staff, and families to determine the child’s type and level of support needs and support access to additional resources as needed.
- Support families to articulate and advocate for their child’s needs with community organizations and government service providers.
- Complete observations, informal assessments, daily notes, and written reports as required and as appropriate to the level of training and knowledge.
- Evaluate the children’s progress, review the level of support needs and individual plans and ensure that services are coordinated, appropriate and accessible.
- Ensure appropriate transportation systems are in place for the student to attend school/childcare.
- Review action plans on a weekly basis and provide weekly/daily (if immediate action is needed) feedback to the Education Director and/or the K-12 Education Manager on necessary action for the benefit of shíshálh Nation children.

One-on-One support
- As required and appropriate, work effectively within an integrated team to support children with extra support needs to fully participate in a childcare, school or out-of-school care setting.
- Supports the child at free-play, group, story, gym, outdoors, snack, lunch, field trips, and nap times when needed.
- Plan, implement and evaluate daily activities and experiences to meet goals and objectives for children with extra support needs.

Program Administration
- Utilize Mustimuhw to develop and maintain current, accurate and confidential participant files that reflect assessment, services, and outcomes.
- Ensure established participant intake procedures are followed.
- Ensure the program’s data collection and management practices align with shíshálh Nation’s legal and ethical obligations to maintain the privacy of children, their families, and staff.
- Works with K-12 Education Manager to develop annual work plans and assists in reporting as required; meets regularly with the Manager with respect to program and operational issues and plans.

Community Relations and Outreach
- Initiate and maintain contact with school staff, relevant community partners, families, groups, and networks as appropriate to the role.
- Attend, participate, and/or facilitate relevant meetings, conferences, or workshops as a representative of the program as required.
- Organize and/or participate in community-based learning about early years, K-7 education and family support.
- Research appropriate agencies for relevant referrals, resources, and available community programs.

**Qualifications, Skills and Abilities**:

- Minimum of Early Childhood Education Certificate and/or Education Assistant Certificate (specialized training in Special Needs, and/or diploma or degree an asset).
- Minimum two years experience as an Early Childhood Educator/Education Assistant in a classroom or one-on-one setting.
- Preferred work experience providing coordination/supervision in an early learning or K-7 environment.
- Knowledge of needs specific to First Nation populations in early care and learning.
- Knowledge of early care and learning theories, wise practices and emerging trends, social determinants of health, and supportive, inclusive, preventative programming.
- Knowledge of key guiding documents, including First Peoples Principles of Learning, BC Aboriginal Head Start Principles and Guide
