**Overview** **Duties** 1. Coordinates with administration, medical and nursing staff and staff from other related departments in planning social services to assure programs can be properly maintained to meet the needs of the residents. 2. Maintains and coordinates the resident waiting list in consultation with the Winnipeg Regional Health Authority, Long Term Care Access Centre. 3. Completes a Pre‑Admission Assessment of prospective residents, at times with Director of Resident Services or designate, gathering information from all sources, to ensure the applicant meets the admission criteria for Park Manor Care and our ability to safely and effectively meet the applicant’s health and behavioral needs. 4. Initiates and maintains contact with applicants, families and other agencies involved with those on the admission waiting list. 5. Coordinates the admission of appropriate applicants, in consultation with the Director of Resident Services, the Chief Executive Officer and Park Manor’s Interdisciplinary Team. Also coordinates the transfer or discharge planning of residents as necessary. 6. Serves as resident advocate and provides resident and family counseling. Supports the physical, social, psychological and spiritual needs of each resident. 7. Reassesses continually the changing needs of residents, attempting to identify, resolve and prevent problems with a team effort. 8. Counsels and advises Park Manor resident families, Park Manor residents and community members by referring them to the appropriate social service & health care agencies when Park Manor Care is unable to provide the services requested. 9. Develops and implements policies and procedures to identify medically related social and emotional needs of the residents. Involves the resident, family and staff when possible. 10. Determines, coordinates and communicates resident needs to staff, other departments, agencies and families concerning the social aspects of the residents' care program. 11. Coordinates and participates in Resident Care Conferences, the End of Life Program, and the Improvement Advisory & Ethics Council and as facilitator of the Resident/Family Council. 12. Maintains up‑to‑date records of pertinent information regarding each resident, charting in the "Progress Notes" section of the residents' charts as necessary.
