**Job Overview**

**Duties**
- Conduct assessments of patients’ social, emotional, and environmental needs, especially within pediatric and hospital contexts
- Develop individualized care plans in collaboration with medical teams and families
- Provide counseling and emotional support to children and their families facing health challenges
- Assist with case management, including coordinating services, resources, and follow-up care
- Collect and document data related to patient progress and social determinants affecting health outcomes
- Support patients in navigating healthcare systems, special education programs, and community resources
- Maintain accurate records using medical terminology and adhere to confidentiality standards
- Participate in multidisciplinary team meetings to discuss patient care strategies
- Engage in ongoing research related to patient care practices and social work interventions

**Qualifications**
- Experience working with adult population.
- Strong communication skills with the ability to advocate effectively for client
- Compassionate approach with a commitment to providing holistic support
- Ability to work collaboratively within a multidisciplinary healthcare team
- Relevant licensure or certification as required by local regulations

Pay: From $50.00 per hour
