Mackenzie Health Long Term Care Facility

RESPONSIBILITIES
- Implement and plan under the supervision of the Resident Program Manager
- Actively and effectively facilitate programs
- Complete resident assessments and establish goals and interventions based on resident needs
- Complete documentation related to programming, including but not limited to assessments, attendance, progress notes, care plans and quarterly reviews
- Support and effectively utilize volunteer resources
- Ensure effective communication with all residents, family members, staff and volunteers
- Work collaboratively as part of a multidisciplinary team
- Other duties as assigned by Manager

QUALIFICATIONS
- Successful completion of a related Recreation, Activation or Gerontology program, diploma or degree required
- Demonstrated oral and written communication skills
- Ability to work independently and complete assignments in a timely manner
- Knowledge and experience RAI-MDS
- Flexible and adaptable
