**Description**

We are currently hiring part-time (approx. $20 - $23 per hour) Client Care Coordinator to work At Your Home Compassion Care.

**Responsibilities**
- Manage home care services with case intake, admission, and care coordination.
- Develop, implement, and evaluate individualized care plans tailored to each client's specific needs.
- Serve as a primary point of contact for clients, staff, and healthcare professionals to ensure clear communication and understanding of care needs.
- Conduct assessments of clients' needs to recommend appropriate services and interventions.
- Monitor and supervise care staff to ensure compliance with agency policies and quality standards.
- Facilitate regular follow-up visits and evaluations to track client progress and satisfaction with services provided.
- Address and resolve any issues or concerns raised by clients or caregivers promptly and effectively.

**Requirements**:

- Post-secondary education in nursing, social work, or a related field.
- Minimum 2 years of recent experience in community health or a related field.
- Knowledge of the healthcare delivery system and community resources.
- Case management experience or recent home care management experience.
- Excellent communication, interpersonal, problem-solving, and decision-making skills.
- Ability to work independently and as part of a collaborative team.
- Familiarity with health care regulations, standards, and best practices.
- Additional language skills are an asset.
- A valid driver’s license and access to a reliable vehicle.
