We are excited to announce an opportunity for an experienced Registered Social Worker, Registered Psychotherapist, Registered Occupational Therapist, or Registered Mental Health Nurse to join our mental health team. The Georgian Bay Family Health Team (GBFHT) provides a host of primary care services including Lung Health, Diabetes, Healthy Aging, Nutrition Counselling, Cardiovascular Rehabilitation and Ontario Telemedicine Network (OTN).

Our mental health team provides exceptional primary mental health care, across the lifespan, to patients of the 52 area physicians who comprise our Family Health Organization.

**Duties & Responsibilities**

The successful applicant will work collaboratively with the Access Coordinator/Team Lead to provide intake, triage, stabilization, care coordination, systems guidance, and transition support. The primary objective of this role is to provide a seamless approach to primary mental health care and to facilitate efficient, effective, and appropriately paced system navigation support. This role will be balanced with individual support, groups, and patient/family mental health education.

**Education and Skills**
- Master’s degree in counselling, occupational therapy, nursing, social work, or psychology.
- Current registration and a member in good standing with the Ontario College of Social Workers, College of Registered Psychotherapists of Ontario, College of Nurses of Ontario, Ontario College of Occupational Therapists, or College of Psychologists of Ontario.
- Minimum of 5 years mental health, counselling and/or psychotherapy experience.
- Capacity to provide support across the lifespan including children, youth, parents, young adults, families, couples, and older adults.
- Excellent interpersonal, communication and networking skills required.
- Excellent organizational and time management skills required.
- Familiarity with EMR PS Suite is an asset.
