Your Opportunity:
Brooks LTC/Sub-Acute care is a combined 24 bed nursing unit providing care to 15 LTC residents and 9 transition patients who are either receiving rehabilitation services to return home, or transitoning to a new living environment. As a member of the CoAct team, the Health Care Aide provides resident and patient centered care according to AHS policies and Continuing Care Health Service Standards. You will collaborate with the interdisciplionary team, resident/patient and thier families, providing wholistic care, by supporting residents and patients with activities of daily living while optimizing their independence. You will complete the resident’s scheduled RAI MDS tracking tool with the team and participate in the development of the resident’s care plan. You will observe, monitor and report any changes in the patients/residents physical, mental, and emotional state to the Registered Nurse. Brooks is one of fastest growing communities in Alberta with a multi-cultural flavor unique to other municipalities. Residents enjoy a wide range of indoor and outdoor recreational activities including camping, boating, fishing, sailing, kayaking, water skiing (water sports) ice fishing and skating in winter, just to name a few.
Description:
As a Health Care Aide (HCA), you will work as a collaborative member of the multi-disciplinary team providing personal assistance including supporting activities of daily living and support services to patients (clients, residents) who require short-term assistance or ongoing support, in accordance with the patient care plan and facility policies and procedures. In this position you will work closely with patients, families, and caregivers spanning the continuum of care in home, community or health care facilities assisting nurses with the provision of routine care to meet patients' hygiene, nutrition, mobility, recreation and safety needs, assisting with organized activities or programs and observing and reporting on a patient's mood, health, and wellbeing. Working under a regulated health professional you will be assisting in the maintenance of a safe environment for patients, staff and visitors, and maintaining effective communication with patients and health professionals.
