Southbridge Care Homes owns and operates several long-term care and retirement homes throughout many urban and rural areas across Ontario.

**Responsibilities**:
- Plan, organize, direct and supervise the department program which includes recreational, restorative care, pastoral care, social work, music therapist and volunteers.
- Implement and evaluate program relating to the five “P.I.E.S.S domains”
- Coordinate therapeutic program development and delivery in collaboration with those of other departments
- Asses departmental needs and draft an action plan.
- Complete Scheduling & payroll
- Recruit/ coach, mentor and supervise departmental/programs staff
- Develop departmental goals and objects.
- Actively participate in program delivery.
- Develop, implement, and monitor programming budget.
- Create and update Resident Health Care Record as they pertain to the department (MDS experience an asset)
- Coordinate the completion of MDS assessments on all residents whiting established timelines
- Education Lead
- Actively participate in the homes’:
- IDTCs (Inter Disciplinary Team Conference)
- PACC (Professional advisory care conference)
- QIP meeting (Quality Improvement Plan)
**Qualifications**:
- Post-secondary diploma or degree in recreation and leisure studies, therapeutic recreation, kinesiology or other related field from a community college or university required
- Knowledge of the concepts of Restorative care, Holistic care, Activation and Dementia Care
- Minimum one year supervisory experience
- Experience in planning, developing and evaluating effectiveness of individual and group programming for frail, elderly residents using an interdisciplinary approach
- Demonstrated oral and written communication skills and have a command of the English and French language.
- Ability to work independently and complete assignments in a timely manner
- Knowledge of community resources, program development, MOHLTC standards
- Previous supervisory experience in long term care an asset
- At least one year of experience with seniors in an organized program or one year of experience with persons in a health care setting required
