**_Transitions_** is looking for a creative, innovative and energetic person to become a **Team Lead** **for Residential Supports**.

A **Team Lead** is responsible for supporting the leadership team in their effort to achieve Transitions’ Vision and Mission statements. Through commitment and coaching, the Team Lead will assist Direct Support Professionals in aligning their support with our values.

This is a full-time Salary position, Monday - Friday 8:30 a.m. - 5:00 p.m. (Must be flexible to work occasional evenings and weekends.)

**JOB RESPONSIBILITIES**:
**Leadership and Support to Direct Support Professionals**:

- Understands and supports Transitions’ Vision and Mission statements.
- Is knowledgeable about Transitions framework and has an understanding of the strategic/operational plans that guide the services we provide.
- Is professional, honest and ethical at all times.
- Coaches and provides direction to team members.
- Deals effectively with crisis; assessing the risk and acts accordingly.
- Attends and participates in Team Lead meetings.
- Completes staffing schedules.
- Chairs team’s meetings.
- Identifies and shares with team members, resources and services within the community.
- Participates in the recruitment process. Identifies staffing needs with Supervisor and may be involved in the interviewing and selection process for staffing.
- Oversees the in-house orientation process.
- Contributes and participates in team member’s performance appraisals.
- Participates in the On-Call rotation (approximately once every 6 weeks).
- Is trained in all aspects of personal care needs and is able to teach and support staff in this area. Is able to provide personal care if needed.
- Complies with all workplace health and safety rules and guidelines.
- Identifies teams’ training needs with Supervisor and helps to create a plan to address the needs.
- Other duties as assigned.

**Communication, Reporting and Documentation**
- Communicates in a professional manner and ensures confidentiality at all times.
- Ensures all required reports are completed accurately and on time:

- Medication errors
- Incident reports
- Ensures staff timesheets and expense forms are completed accurately and submitted to payroll on time.
- Works in conjunction with the person supported and their guardian/family member on goal setting.
- Checks and replenishes all safety and emergency supplies.
- Completes annual, monthly and quarterly updates on Personal Profiles - Support Needs
- Consents
- Service Agreements
- Individual Service Plans (monthly ISP goal updates)
- Quarterly report - complex needs
- Ensures personal information is updated and distributed correctly:

- Supervisor - Master File
- Payroll - Electronic updates
- On-Call - Weekday and Weekend
- Other duties as assigned

**Qualifications**:

- Diploma in Disability Studies or related area of study. Previous direct or related experience may be considered equivalent.
- A minimum of 2 years’ experience in the disability or related field of work.
- Strong organizational, problem solving and interpersonal skills.
- Ability to effectively communicate (both verbal and written).
- Previous work experience in a not-for-profit organization would be an asset.
- Must have a clear, current Criminal Record Check, which includes Vulnerable Sector.

**Job Types**: Full-time, Permanent

**Salary**: $22.55-$25.37 per hour
