**Ontario Health atHome** is looking for an experienced leader for our Patient Services team. The successful applicant will be reporting to the VP of Patient Services, the **Director** is responsible for leading the delivery and implementation of patient care programs within a defined region of Ontario Health atHome. This role combines clinical expertise with strategic leadership, overseeing program development, quality standards, budgeting, and team performance. The Director will build strong partnerships across the health system to ensure integrated, high-quality, and patient-centered care.

What will you do?
- Provide strategic leadership and oversight for Patient Services programs within a designated portfolio and region of Ontario Health atHome, ensuring alignment with provincial priorities and excellence in care delivery.
- Lead, manage, and support a team of Managers, including staffing, budgeting, mentorship, and performance management, to meet organizational and patient care goals.
- Monitor daily operations and service delivery to ensure adherence to policies, clinical best practices, and quality standards, taking corrective action as needed.
- Oversee the development, implementation, and evaluation of operational and clinical programs, processes, and initiatives, with a focus on continuous improvement, patient safety, and community responsiveness.
- Manage financial performance of the portfolio, including identifying budget requirements, addressing variances, and ensuring cost-effective service allocation.
- Resolve complex service issues and provide expert guidance on patient care matters, risk management, and emerging needs across the care continuum.
- Lead or support change management, staff education, and training initiatives related to new programs, technologies, and policies.
- Develop and maintain strong partnerships with internal and external stakeholders, including hospitals, service providers, and community agencies, to ensure integrated, patient-centered care.
- Represent Ontario Health atHome on committees, working groups, and projects to advance system-wide quality, integration, and innovation.
- Promote a positive, inclusive workplace culture that aligns with the organization’s mission, vision, values, and commitment to equity, diversity, and anti-racism.
- Perform other duties as required.

What must you have?
- Minimum 5-7 years of healthcare experience, including 3-5 years in progressive leadership roles, with a strong background in managing patient service programs in community care settings.
- University degree in a healthcare-related discipline required; Master’s degree preferred.
- Current membership in good standing as a Regulated Health Professional (RHP), with ongoing maintenance of professional credentials.
- Possess strong critical thinking skills with a positive solution focus
- Strong strategic and operational planning capabilities, with in-depth knowledge of Ontario's healthcare system, including Ontario Health atHome programs and services, MOH, MLTC, MCCSS, and community health agencies.
- Proven leadership in managing multi-sector patient services initiatives, with expertise in facilitation, negotiation, problem-solving, and driving innovation and continuous improvement.
- Deep understanding of case management, transitional care issues, service provider contracts, and the evolving role of Ontario Health atHome within the broader healthcare landscape.
- Demonstrated ability to design and implement effective processes across complex systems involving multiple stakeholders.
- Knowledge of community resources, healthcare provider roles, and system integration best practices.
- Strong communication, conflict resolution, and presentation skills.
- Exceptional ability to build and maintain collaborative relationships with internal teams and external partners across healthcare settings (e.g., hospitals, long-term care, community programs).
