The Complex Case Coordinator provides complex care coordination to vulnerable and

chronically homeless youth within the shelter through assessment, planning, facilitation, care

coordination, evaluation and advocacy. The Complex Care Coordinator empowers youth, draws

on their strengths and capabilities, and promotes an improved quality of life by facilitating timely

access to the necessary supports that help reduce the risk of homelessness.

**Responsibilities**:

- Working from a trauma informed framework and strengths-based perspective, provide complex

care coordination through the development of a system of care for complex youth based on

their unique needs.
- Receive assignment of identified youth from Home for Good (HFG).
- Provide case management with youth in developing and following through with comprehensive

case plans designed to concentrate on obstacles preventing success in housing as well as

promoting longer term stability and achievement.
- Utilize culturally appropriate interventions as required to design care plans and make referrals

to enhance systems of care.
- Manage individual progress of youth case plans and follow-up to ensure interim goals (daily,

weekly) are still suitable and are being met.
- Conduct weekly in-person check-ins with active clients to ensure goals are being worked on

based on the assessment.
- Collaborate with staff to help client’s secure affordable and appropriate housing using a

Housing First for Youth model and to ensure wraparound services are being offered through

outreach, mental health, harm reduction and housing.
- Employ the appropriate outcome measurement tools to provide therapeutic redirection and

motivation to the youth as well as capturing data for program appraisal and development.
- Accompany youth to viewings and meetings when necessary and act as an advocate/support

person.
- Demonstrate skill and situational awareness when engaging and/or transporting youth in the

community to maintain appropriate safety precautions.
- Support youth with housing and location orientation by creating and updating ongoing

development of their neighborhood to connect to local services and decrease social isolation.
- Provide life skills teaching to help build client capacity to maintain housing, especially focusing

on tenancy skills and knowledge.
- Conduct assessments, utilize care planning tools, facilitate care coordination, and advocate for

options and services to meet youth’s comprehensive needs in order to achieve greater life

stability.
- Promote and encourage youth engagement and positive youth development in care path

planning and establish goals by fostering therapeutic relationships.
- Facilitate open communication with landlords/building superintendents to ensure positive

relationships and resolve immediate concerns or issues.
- Form associations and relationships with a broad spectrum of community service agencies to

be able to refer youth. Initiate contacts with or responds to contacts from a wide variety of

individuals on matters concerning youth and their plan.
- Develop relationships with primary health/community health providers to ensure youth have

access to health care and makes referrals to other professional supports depending on

individual needs.
- Perform all other duties as required to ensure the successful management of shelter

operations while partnering with all stakeholders in support of the organization’s success.

Administrative and Other Related Activities
- As an employee of Our Place Peel, abides by all the policies and procedures of the agency.

Has knowledge of and ability to carry out all safety requirements and emergency procedures.
- Compile statistical information and prepare monthly reports as required for program evaluation

and development and track key performance indicators.
- Maintain accurate records, transition and service restrictions and enter client data for monthly,

quarterly and annual funding reports.
- Participate as part of a coordinated service Housing First Model.
- Maintain confidential files on residents and ensures all appropriate documentation is

completed.
- Participate on committees and task forces with other agencies addressing issues facing youth.
- Carries out other related duties as deemed necessary by Management.
- Other duties as assigned.

Qualifications:

- BSW or equivalent degree in an accredited human service / social service program. A

combination of education and experience will be considered.
- Experience supporting youth through evidence-based practices and frameworks

(Motivational Interviewing, CBT, DBT, Harm Reduction).
- Cultural competency sensitivity and commitment to working through an anti-oppressive, antiracism

framework.
- Knowledge of housing first for youth, trauma informed care, and harm reduction practices an

asset
- 3+ years’ experience supporting youth experiencing chronic homelessness, mental illness

and substance usage in obtaining ho
