**Your Opportunity**:
The Hamilton Family Health Team is one of the largest Ontario Health Teams in the province.

The **MAiD Coordinator**will have responsibility for coordinating and facilitating MAiD services for patients living in Hamilton, who have requested MAiD support.

**About Medical Assistance in Dying (MAiD)**

MAiD is supported through the following changes: _“In February 2015, the Supreme Court of Canada ruled in Carter v. Canada that parts of the Criminal Code would need to change to satisfy the Canadian Charter of Rights and Freedoms. The parts that prohibited medical assistance in dying would no longer be valid. The Supreme Court gave the government until June 6, 2016, to create a new law._

**Roles and Responsibilities**:

- Answering phone calls, providing support to patients and caregivers who may have questions or wish to self-refer.
- Accept referrals, triage patients and liaise with team assessors/providers.
- Maintain a regular caseload, estimated 12-15 open cases at any given time.
- Provide supportive health teaching for patients and caregivers relating to MAiD eligibility criteria and processes.
- Coordinate, attend and participate in patient assessments with team a team including physicians and/or Nurse practitioners.
- Coordinate and assist with all aspects of provision (i.e. organize IV starts, drawing up of meds prn, preparing patient and family etc.)
- Support patients with resources should they wish to donate their organs.
- Maintain accurate documentation (SOAP notes, MOH forms, outcome tracking etc.),
- Foster and maintain collaborative relationships with Primary Care/referring MRP.
- Liaise with community partners (e.g. HCCSS, visiting nurses) as needed.
- Provide back-up coordination coverage for team members as needed.
- Participate in quality and performance improvement activities
- Participate in community tables and planning committees
- Participate in sustainability planning for the team, considering innovative options to ensure long-term viability of the team.

**If so, we are looking for a future colleague who has**:

- Registered Nurse or Registered Practical Nurse in good standing with the College of Nurses of Ontario with experience and expertise in intravenous starts.
- Or Registered Social Worker with experience supporting patients through MAiD and strong knowledge of a palliative approach to care.
- Experience providing a empathetic approach to care and supporting both patients and their caregivers.
- 3-5 years of clinical experience preferred
- Strong understanding of all MAiD legislation and processes
- Strong knowledge of relevant legislation including but not limited to Freedom of information and Protection of Privacy Act, Regulated Health Provisions Act.
- Exceptional and compassionate interpersonal and communication skills
