The Family Support Worker plays a vital role in providing direct client support, assisting families with service coordination, and advocating for client needs to ensure a safe and supportive environment.

**Job Summary**

The Family Support Worker is responsible for providing direct care, implementing individualized support plans, and facilitating community connections for clients and their families. This role involves collaborating with families, assisting with behavioral management strategies, and ensuring smooth transitions for clients across programs. Family Support Workers work closely with the Family & Practice Supervisor, Program Supervisor, and Office Supervisor to ensure high-quality services, compliance with CARF accreditation standards, and strong family engagement.

**Key Responsibilities**

**Direct Client Support**
- Provide hands-on support to children, youth, and adults in respite, supported living, or community-based programs.
- Assist with daily living activities, including personal care, meal preparation, and medication support.
- Implement behavioral support plans and crisis intervention strategies when needed.
- Encourage social engagement, skill-building, and independence among clients.
- Accompany clients to community outings, medical appointments, and activities.

**Family & Community Engagement**
- Collaborate with families to understand individual client needs and preferences.
- Provide emotional support and advocacy for clients and families navigating support systems.
- Assist in connecting families with external resources, FSCD services, and funding options.
- Facilitate family meetings to discuss progress, concerns, and service adjustments.

**Behavioral Support & Crisis Management**
- Implement de-escalation techniques and positive behavioral interventions.
- Provide crisis response and emotional regulation support when needed.
- Document incidents, behaviors, and interventions according to policies.

**Compliance & Documentation**
- Maintain detailed client progress reports and case notes.
- Ensure compliance with FSCD, PDD, and CARF accreditation requirements.
- Participate in case reviews and program assessments to improve service delivery.

**Qualifications & Skills**
- Education: Diploma or degree in social work, disability studies, human services, or a related field (preferred).
- Experience: Minimum 1-2 years of experience in direct client support, respite care, or community services. Certifications:

- First Aid/CPR (required or willing to obtain).
- Non-Violent Crisis Intervention (preferred). Skills:

- Strong communication and interpersonal skills.
- Ability to work in fast-paced environments and respond to client needs proactively.
- Familiarity with FSCD, PDD, and community support programs.

**Job Types**: Full-time, Part-time, Casual

Pay: $21.00-$23.00 per hour
