The Kemptville District Hospital is seeking a permanent part-time Social Worker/Discharge Navigator to join our team.

- Bachelor’s degree in social work (BSW) or equivalent required;
- Current certificate of registration in good standing with the Ontario College of Social Workers and Social Service Workers;
- Relevant clinical or discharge planning experience in a healthcare setting;
- Certification in Cognitive Behavioral Therapy, Motivational Interviewing, and Mindfulness-based stress reduction, preferred;
- Ability to work effectively in a multi-disciplinary team environment;
- Flexibility to adapt to organizational priorities and client needs.
- Demonstrated comprehensive knowledge of community services, agencies, and placement facilities;
- Demonstrated knowledge and understanding of legislation and acts applicable to patient discharge and transition (e.g. Health Care Consent Act, Mental Health Act, Public Hospital Act, Substitute Decisions Act);
- Demonstrated knowledge of financial aid available from federal, provincial, and municipal programs;
- Strong communication, organizational, and problem-solving skills;
- Respect for individuals of diverse cultures and demonstration of culturally sensitive practice.

**Please note**: In order to work in a healthcare facility, _the Public Hospitals Act 1990_, _Regulation 965_ requires employees to provide proof of up-to-date immunization/vaccines for the following diseases: Tuberculin Skin Testing (TB), Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox), Hepatitis B, Tetanus/Diphtheria (DTap), Influenza (during flu season) and two COVID shots plus a booster shot. This proof must be provided to our Occupational Health Office in order to be cleared to work.

Pay: From $38.93 per hour

Expected hours: 15 - 22.5 per week
