**Position Purpose**: Working with the guidance and support of the Community Health Nurse, the Pre/Post-Natal Support Worker provides education, support & resources to all pregnant and post-natal families with infants up to 12 months of age. This will include education and information to adolescents and to those planning a pregnancy.

**Job Duties and Responsibilities**:

- Meet with all newly pregnant women/families and complete registration and intake form into the CPNP/Prenatal Program.
- Programing will be guided by Nak’azdli Language, Traditions and Culture
- Complete and review 72-hour dietary intake forms with all pregnant families.
- Ensure pregnant families have a family practitioner. Work as a team, including the Community Health Nurse & the Family Practitioner in order to provide a comprehensive care plan.
- Assist and accompany women to appointments as needed and appropriate.
- Assess women for daily living challenges such as PPD, anxiety, addictions, nutrition, and provide appropriate referrals.
- Assist women to access resources such as financial support, Social Worker, housing, health care, child care, Jordans Principal
- Provide one-on-one sessions with pregnant and post-natal women/families.
- Offer home support and weekly follow up of post-natal families & babies for the first 4 weeks following delivery.
- Attend initial home visit with the CHN assigned to the family.
- Provide family with a Baby Welcoming Gift.
- Provide Regular Home Visits Postnatally
- according to care plan.
- Support the new family with Wholistic care to birth recovery.
- Support and promote breastfeeding in families and community.
- Pre/Post-natal group sessions
- Offer Prenatal Education Classes every 2 months alternating with Early Pregnancy Classes and Labor/Delivery Preparation.
- Manage and distribute bi-weekly grocery cards including Nutrition Information/Teachings
- Work with families on healthy food choices & budgeting.
- Assist with planning and implementing of Annual Baby Welcoming, Safety Fair and participate in other community wide events as appropriate.
- Promote the Pre/Post Natal Program in the community.
- Meet Charting requirements
- up to date and relevant.
- Meet Reporting Requirements
- Attend Meetings relevant to program
- including interagency.

**Assets**:

- Child Birth Educator
- or willing to complete the course
- Breastfeeding Educator
- or willing to complete the course.
- Trauma Informed Care
- or willing to complete training.

**Requirements**:

- Pre/Post Natal Experience
- Privacy and Security Understanding
- Satisfactory Criminal Records Check
- Class 5 Driver’s License
- Food Safe Certified
- Proficient with technology and office equipment
- Proficient with social media and posting on program sites.
- Strong Communication Skills
- Partnership, Collaboration and Advocacy Skills
- Ethics and Professionalism

**Hours of Work**

Your regular hours of work will be from 8:00am - 4:00pm for 7 hours per day from Monday to Friday and it is expected that all work will be completed within your regular working hours, however, you may be required from time to time to work outside your normal hour of work and flex time accordingly.

**Working Conditions**
- Ability to Conduct in Home Visits
- Ability to attend meetings and conduct presentations.
- Extensive computer usage and sitting for long periods of time.
- Manual dexterity required to use desktop computer and peripherals.
