Community Disability Support Worker in Swan Hills-26hrs a Week

**Position Summary
- in Swan Hills Tues-Fri 1pm-7:30pm**
Reporting to the Team Leader, the Community Disability Services Worker is responsible for providing support to Individuals in accordance with Blue Heron’s philosophy, the Principles of Support and the Individual’s Personal Support plan. The Community Disability Services Worker provides support that facilitates the Individual’s growth, development and achievement of personal goals.
The Community Disability Services Worker works closely and in consultation with the Team Leader, Program Coordinator and the Individual’s support team.
1. Specific
**Responsibilities**:
a. Individual Support
- Promote the interdependence of the Individual.
- Act as a role model, displaying professional attitudes and behaviour.
- Encourage a developmental approach with Individuals in an age and situation appropriate manner.
- Encourage community involvement for each Individual, providing opportunities for structured and informal involvement.
- Provide appropriate support to each Individual, adhering to ethical standards outlined in policy.
- Participate in professional appointments as requested/required, i.e. medical, dental, and optical, are kept and documented.
- Promote the Individual’s health and well-being, e.g., exercise, healthy meal choices, management of stress.
- Promote safe and healthy environments in the Individual’s home, community and employment.
- Relay important concerns and information to the Team Leader. Program Development and Implementation
b. Program Development and Implementation
- Participate in the person-centered planning process for each Individual, including providing updates, assessment information and any and all other relevant information.
- In consultation with the Team Leader and/or Community Disability Services Practitioner, participate in the development of programs for the Individual.
- Implement all programs in an environmentally and situation appropriate manner, following identified procedures.
- Complete all documentation as required for each individual program procedure.
- Complete and submit monthly, or more often upon the request of the Team Leader, written program updates regarding the Individual’s progress.B1-3 - Community Disability Services Worker Personnel File: Part: 1.1 Latest Revision: June 2021 Page 2 of 4
c. Community Inclusion
- Represent themselves, the program, and the association professionally and positively within the community as a whole.
- Present Individuals in a positive, respectful manner, maintaining confidentiality.
- Utilize community resources to promote a positive service profile and to research opportunities within the community.
- Participate in an effective working relationship with the community to promote and facilitate Individuals obtaining employment, recreation, leisure or living within the community.
d. Home and Community Support
- Monitor and/or administer medications, as required.
- Follow daily routines in a respectful manner, ensuring Individuals are as involved as much as possible.
- Ensure the individual’s home is maintained in a safe and healthy manner.
- Ensure all equipment and appliances are in good and safe operating condition.
- Support the Individual in developing independent living skills
- Support the Individual in making safe and healthy choices
- Utilize community resources to enhance services to the Individual
- Promote the development of the Individuals support network
- Support the Individual in building personal connections in the community
e. Job Development/ Job Matching/ Job Coaching
- Work collaboratively with the Career and Employment Coordinator to support individuals’ goals of employment and in the following areas:
- Provide volunteer or pre-employment support to Individuals seeking employment.
- Provide employment support and job coaching to promote on-the-job success to those Individuals seeking competitive employment.g. Documentation
- Complete, and keep confidential all documented records and files as per program and policy requirements.
- Ensure all required documentation, including daily log notes, incident reports, medication documentation, data collection, program updates, financial records and reporting, monthly and quarterly reporting etc. are completed and maintained as per policy.
- Keep all documentation safe and secure.
h. Teamwork
- Attend all team meetings, sessions and general staff meetings.
- Work effectively with team members in a positive and professional manner.
i. Liaison
- Establish and maintain effective and professional relationships with the Individual’s personal supports, including the individual’s guardian, family members, friends and natural community connections.
- Establish and maintain effective, open and professional working relationships with other Blue Heron employees working in support of the Individual.
