Job Description:
Scarborough Health Network is in the midst of an exciting transformational journey. The Planner is responsible for assisting specific programs to plan and design innovative models and services to meet the changing needs of the diverse community in alignment with SHN’s strategic plan. This role will be of interest to individuals with strengths in quantitative and qualitative data collection and analysis, stakeholder engagement, best practice review and co-design. This position will have a primary focus on the SHN Palliative Care Program and a secondary focus on program partnerships.
- The Palliative Care program has been recognized as one of SHN’s Specialty Services, committed to providing coordinated and integrated care for the Scarborough community across all settings and transitions in care.
- Partnering with the Scarborough Palliative Care Collaborative (SPCC) will be essential as this network of Scarborough palliative care advisors and provider organizations continue to work on the development and demonstration of a fully integrated seamless system of palliative care for Scarborough.
Responsibilities:
The Palliative Care program at SHN is engaged in significant transformation initiatives including community integration, innovative models of care, enhanced community-based services, collaborative partnerships and more. To advance this transformational work, the Planner will:
- Work closely with program leadership to identify planning goals and parameters
- Facilitate the development and implementation of a program planning process utilizing established planning methodologies
- Engage stakeholders to assess the current state and articulate the desired future state for the program to optimize the patient/family experience
- Conduct quantitative analysis and interpret/present results including service utilization data, volume trends and demographic data
- Develop and implement an inclusive stakeholder engagement strategy to gather qualitative information using various consultation and facilitation methods; plan and support consultation events and activities to involve staff, patient/family advisors, community members and other service providers
- Conduct structured analyses, utilizing multiple data sources to assess and forecast opportunities to grow, augment or expand current practices
- Identify program interdependencies and partnership opportunities and collaborate with various departments and external stakeholders as needed
- Identify relevant best practice models and industry trends
- Support program-specific local and regional initiatives in collaboration with other partners including Scarborough Ontario Health Team
- Align to Accreditation Canada standards related to planning and service design
- Prepare written documents to support the planning process (e.g. surveys, data collection guides), report findings, describe models and communicate final plans
- Present analyses and communicate the significance of results to key stakeholders
- Develop monitoring and evaluation plans
- Develop change management plans
- Support implementation plan development
- May perform other related duties as required
Requirements:
- Masters degree or equivalent in health, planning or related discipline
- Minimum 3 years of experience in a health care planning, strategy or project management role
- Demonstrated experience in program planning methodologies
- Expertise in the collection and analysis of quantitative data and the gathering of qualitative information for planning purposes
- Facilitation skills to engage diverse stakeholders such as patients, health care workers and community members
