The Alzheimer Society of Muskoka (ASM), serving the communities of Muskoka and Parry Sound, is dedicated to improving the quality of life for individuals living with Alzheimer’s disease and other dementias and their caregivers. We are committed to providing advocacy, a caring and understanding support system, individual, family and group counselling, public education.

The Alzheimer Society of Muskoka offers a wide range of programs and services addressing the progress of Alzheimer disease with support for caregivers and families including supportive counselling, public education, weekly drop in programs, in home recreation, a resource lending library, music and Minds in Motion.

**JOB SUMMARY**:
The In-Home Recreation Coordinator (IHRC) will create recreation intervention plans that provide clients with an opportunity to participate in a range of activities that are stimulating, therapeutic, and socially engaging in their own home. The IHRC also matches trained volunteers with clients to engage in meaningful activities within their own home setting.

In partnership with the Community Engagement Coordinator and Recreation Support Facilitator, the IHRC is responsible for screening, training, and supervising program volunteers and post-secondary student learners who are supporting program delivery.

**KEY RESPONSIBILITIES**:

- Process client referrals and facilitate the matching process with appropriate volunteer and family
- Create recreation intervention plans for volunteers to execute with matched client using a holistic approach, providing activities in keeping with the individual’s cultural diversity, recreational interests, activities of daily living with the therapeutic recreation domains as the overarching goal
- Provide leisure education for care partners and other health care professionals involved in client case
- Update In Home Recreation Training as new education, research and knowledge is gained
- Execute In Home Recreation Training to volunteers monthly and as needed
- Purchase activity items for client and volunteer visits
- Maintain an inventory list of the in-home kit for clients and volunteers
- Manage the caseload of clients assigned to the program
- Provide orientation, training, and feedback for In-Home Recreation volunteers
- Manage a caseload of volunteers who are matched with clients
- Orientate, train and supervise volunteers who are matched with clients
- Provide activities for volunteers to complete with clients on a weekly basis
- Serve as a liaison to the families, providing them with information about the Alzheimer Society of Muskoka and its services, and directing them to the appropriate supports
- Organize and plan for monthly team meetings to discuss caseload and provide an information exchange with volunteers of the In-Home Recreation Program
- Ongoing communication and monthly meetings with the Executive Director
- Work closely with program staff for ongoing assessment of clients for In Home Recreation program
- Collaborate with peer IHRC and work closely with broader Society program staff for ongoing assessments of clients for the In Home Recreation program
- Conduct regular program evaluation by soliciting feedback from clients, volunteer and ASM program staff
- Always represent the Alzheimer Society of Muskoka in a professional manner and act in the best interests of the organization
- Liaise with community partners in conjunction with the program, education, and fund development staff
- As an employee of a non-profit organization, you are expected to participate in fund development activities as required
- Other related duties as requested

**QUALIFICATIONS**:

- Post-secondary degree in Therapeutic Recreation and/or Recreation and Leisure, or gerontology
- Minimum 3 years experience working with persons with dementia and/or their families
- Comprehensive knowledge of and understanding of Alzheimer’s Disease and other related dementias
- Experience building relationships with volunteers, staff, and community partners
- Training and public speaking experience
- Experience working with client reporting and volunteer database
- Knowledge of local Alzheimer Society programs and services
- Familiarity with and commitment to a person-centred approach to service delivery
- Knowledge of relevant community resources
- Ability to effectively resolve conflict which may include the presence of aggressive verbal or physical behaviours
- Excellent interpersonal and communication skills with maturity to deal supportively and tactfully with people affected by Alzheimer’s disease, professionals, community members and volunteers
- Strong written, verbal and listening skills
- Ability to lead and work as part of a team
- Ability to work independently
- Excellent organizational, planning and time management skills, including the ability to multitask
