**General Description**:
The Mi’kmaw Native Friendship Centre is currently looking for One **(1) Intensive Case Manager** to support residents living at the Diamond Bailey Healing Centre. **The** Diamond Bailey Intensive Case Manager (ICM) will be responsible for supporting clients who are living at the Healing Centre to develop an individualized plan based on their goals and long-term housing objectives. Residents living at Diamond Bailey are engaged in personal healing journeys that consider the complex needs associated with a history of chronic homelessness. The Intensive Case Manager (ICM) will be responsible for helping residents follow communal living guidelines, engage in cultural and life skills programming, and maintain their housing at Diamond Bailey. Residents supported by the Diamond Bailey ICM will likely experience complex barriers when trying to maintain their housing and progress into permanent housing related to their physical health, mental health, and substance use with varying levels of ability and life skills. The ICM must support clients with a client-centered, anti-oppressive, and decolonized trauma informed lens with an emphasis on harm reduction practice. The Diamond Bailey ICM will offer culturally safe referrals to resources and supports for Indigenous clients and utilize the programs offered through the Mi’kmaw Native Friendship Centre. This can include referrals to elders, organizing support circles, access to harm reduction supplies, and offering cultural knowledge to clients. This is a designated position for Indigenous applicants. This job description will change as the workplace changes.
- **Key Accountabilities**_
- Participate in agency responsibilities including but not limited to; staff meetings, representing the agency at community meetings, attend bi-weekly case conference meetings, safety and monitoring. Responsibilities include intake, partnership meetings, case conference, training and committees.
- Ensure the program delivery is congruent with the Mi’kmaw Native Friendship Centres mission, vision and philosophy.
- Develop a program plan with individuals that reflects their personalized healing journey while espousing values that uphold the principles of Housing First and harm reduction care.
- Monitor individual progress towards their goals and ensure case notes are updated and well documented.
- Provide daily support to individuals living at Diamond Bailey and offer outreach support to residents who have exited the program.
- Support clients to navigate appropriate systems related to obtaining and maintaining housing
- Input and maintain the HIFIS /MNFC’s database. Documentation must be in accordance with the agency standards and adhere to funder requirements
- Ensure all clients are updated on the By-Name-List.
- Attend all lease signings with clients.
- Ensure clients are fully informed of all programs of the MNFC and the appropriate referral process.
- Advocate for client’s rights at an individual and broader systemic level.
- Assist clients with obtaining items they need with first moving into housing (to include managing purchases and reimbursements as per our finance policy requirements).
- Work in collaboration with the Diamond Bailey Coordinators and MNFC Housing team to provide wrap around supports to residents.
- Work in partnership with both private and non-profit sector of the housing market to ensure appropriate placement for clients.
- Connect clients to community resources, supports and meaningful activities as needed.
- Liaise and develop relationships with landlords and property managers within HRM.
- Maintain confidentiality, privacy and clear professional boundaries with colleagues and clients.
- Compile statistics and data that will assist with the outcomes and evaluation of this housing initiative.
- Commit to the development of the MNFC and participate in internal committees, projects and activities.
- Assist in program and service development to ensure that quality, effective and client-centred programs and services are delivered.
- Adhere to all policies governing the Mi’kmaw Native Friendship Centre.
- May perform other related duties as assigned.
- **Qualifications and Experience Required for the Position**:_
- Diploma / certification in community social services; or education in mental health, addictions, concurrent disorders/ co-occurring disorders and social work.
- Two years of recent, related work experience with an emphasis on mental health, substance use disorders, homelessness and/ or social services.
- Or an equivalent combination of education, training and experience.
