The Transitional Housing Health Services Caseworker will develop and implement health care and harm reduction programming, develop relationships with residents and connect with external harm reduction and health care partners to refer residents as needed. This position will be responsible for the provision of case management services, with a focus on health care, including harm reduction and mental health. This role will collaborate with the transitional housing caseworker and follow-up caseworker to offer a well-rounded support team to both current and former residents, providing education and support to other house team staff who share this case load. We are committed to serving people who face barriers to healthcare and the Health Services Caseworker will focus on reducing these barriers.

RESPONSIBILITIES
- Foster a holistic approach to providing support to residents while developing and implementing health and harm reduction programming
- Coordinate on-site health services with a focus on harm reduction
- Engage in team case management and clinical consultation meetings
- Provide case management, coordination of supports, appointment accompaniments, referrals and advocacy on relevant health issues
- Connect clients with support in the community to help them achieve and maintain stability, including support groups, life skills groups and social events.
- Facilitate referral of clients to specialists, hospital or community resources as appropriate, along with follow-up appointments
- Maintain accurate, up-to-date case notes, progress reports, and records; complete and compile all necessary file notes, reports, statistics and other written documentation on a timely basis and in accordance with agency standards
- Collaborate with case workers and other members of the transitional house team in coordination of support for residents
- Develop individualized plans of action with short
- and long-term goals and identifying strategies to achieve and maintain stability in all avenues of health
- Collaborating with community partners to provide more on-site health services
- Enhance the capacity of internal and external stakeholders by providing timely feedback and coaching to support harm reduction and health promotion engagement with guests
- Assist guests in identifying health goals, ensuring goals are attainable and time specific, and that the client plays an integral role in the decision-making process
- Encourage clients in problem solving, conflict resolution, harm reduction, and medication management as appropriate
- Educate clients regarding emerging and persistent challenges related to addiction
- Model a trauma-informed, anti-oppressive approach to health care and harm reduction practices
- Develop and maintain positive and collaborative relationships with stakeholders and community partners
- Participate in the development, implementation and evaluation of services provided
- Organize group programming that promotes well-being and recovery
- Contribute to the strategic and operational planning process of providing an alternative to relying on emergency services
- Conduct ongoing evaluation of health programs and services delivered
- Advocate, as required, on behalf of residents
- Provide or support the development of educational and awareness sessions, briefings, and resources pertaining to common issues observed while providing services
- Support the overall operations of the Centre; other duties as assigned or requested.

- High levels of trauma content and pressure.
- Intermittent physical activity including walking, standing, sitting, lifting.
- Exposure to infectious waste or illnesses.
- Safety precautions due to proximity to hazardous materials and/or contagious diseases.
- Travel between locations and some scheduled overtime is associated with this position.

QUALIFICATIONS:

- BSW, SSW, Addictions/Mental Health, Health Promotion or another health-related field of study or demonstrated experience in a related field
- Crisis Prevention and Intervention certificate
- CPR, First aid certificate
- Lived experience will be considered an asset
- Ability to set expectations, and monitor progress of processes
