We are currently looking to hire a:
**Health Resource Navigator - Intake Team**
**1 - Full Time, 1-Year Contract (35 hours per week)**
**Reporting To**:Director of Community Support Services
**Purpose**:
As a critical first point of contact, the Centralized intake team is gateway to SCHC’s programs and services. Utilizing their program knowledge, skills and judgment, the health resource navigator will review and prioritize all referrals, respond to program/service inquiries, as well as ensure that the client intake/discharge processes are carried out in a timely, accurate and sensitive manner. The Navigator will also make internal and external referrals, as appropriate, and act as a linkage for clients and patients across SCHC services and the broader healthcare system.
SCHC operates with an integrated and interdisciplinary approach. All team members collaborate across various roles and programs to address the community's needs.

**Key areas of responsibility**:
- The Health Resource Navigator will ensure clients are connected to the right resource at the right time and ensure barriers are reduced wherever possible.
- The Health Resource Navigator will help residents improve their awareness of and access to health promotion services and behaviors throughout their healthcare journey. This will involve providing one-on-one and group education on preventive care topics.
- The resource navigator will use an approach that is supportive, empathetic, open-minded and non-judgmental as they assess/triage client's needs and co-develop solutions.
- The Navigator is responsible for ensuring that care plans are designed with a foundation in culturally sensitive, anti-racist and trauma-informed practices.
- Continuous engagement with clients throughout their journey to ensure effective outcomes based on the clients needs and priorities.
**Educational and/or Professional Qualifications**:
- An undergraduate degree in a health or social science discipline from a recognized university or significant related work experience.
- Allied health professionals is considered an asset.
- Experience with case management, supportive listening, life coaching and goal setting.
- Minimum 3 years experience working directly with diverse communities and vulnerable populations.
- Knowledge of and experience in supporting the Social Determinate of Health.
- Support clients’ awareness of and access to health promotion care and decisions using motivational interviews and brief action planning.
- Facilitate linkages between clients and community resources and collaborating with appropriate service providers or community organizations.
- Identify issues and provide advice, consultation, support and information to clients, their families, service providers and the community on mental health and related matters.
- Familiarity with electronic client records and Microsoft Office.
- Address barriers that impact on the client’s and family’s ability to access health services.
- Participate in providing education to community service providers and other stakeholders regarding culturally responsive service.
- Demonstrated commitment to principles of an anti-oppression and cultural competence framework;
- Excellent interpersonal, communication (written and verbal), problem-solving, organizational and time management skills;
- An interest and commitment to working in a low-income, multilingual, multiracial community;
- Thorough knowledge of broad determinants of health and issues affecting low income, multi-lingual and radicalized and LGBTQ communities;
- Maintain effective and timely documentation of client records.
- Demonstrated flexibility to work in a fast paced, ever-changing environment.
- Ability to speak a second language of our priority populations is an asset.
- Current HCP CPR and First Aid.
