**PARENT COACH CASE COORDINATOR**
Parent Coach Program
Permanent, Full-Time (1.0) EFT Position
Classification/Salary: PCCC -$41,035 - $59,352 per annum
Reporting to the Manager of the Parent Support Program, the Parent Coach Case Coordinator will work with parents and families in the Parent Coach program. The Parent Coach program seeks to strengthen relationships within the family home by empowering parents in their parental role and supporting them to attend to the various needs and demands of the household. The Parent Coach Case Coordinator, using a strength-based, solution-focused approach, will develop a collaborative, helping relationship that is grounded in a spirit of respect, connection, curiosity, and hope. The Parent Coach Case Coordinator will be responsible for completing comprehensive needs assessments, service planning and implementation, along with providing coordination and enhancement of services.
As a representative of Family Dynamics, the person in this position will adopt the mission, philosophy, and values of the agency in all aspects of their work. Clients are to be treated with dignity and respect and assured of confidentiality
**Qualifications**
**Education & Experience**
- Post-secondary degree in Social Work, Inner-City studies, Human Ecology, or other related discipline is required..
- 2 years of experience in direct client services, preferably in a service coordination and case management role is required.
- A combination of education and experience will be considered.
- Experience working in, or thorough knowledge of, Manitoba’s child welfare system.(desired)
- Experience providing in-home supportive service to families. (desired

**Duties and Responsibilities**
SERVICE DELIVERY
- Completion of a comprehensive needs assessment of client strengths and challenges in all life areas.
- Assessment of barriers, priorities, and goals.
- Collaborative service planning.
- Meet regularly with families, primarily in their place of residence, while working towards established objectives.
- Supportive counselling, guidance, problem-solving, skill building, and psychosocial education.
- Continuous assessment of the effectiveness of intervention and follow-up.
- Adjust the area of focus of intervention as necessary.
- Provide support and advocacy on behalf of clients with systems and services such as CFS and schools when needed to help parents feel heard and understood as well as to help clients understand the expectations of the service provider.
- Linkages to community resources and social networks.
- Coordination of services.
- Be accountable to the Program Manager for progress reports and evaluation of services.
- Monitor the wait list and report trends and needs to the Program Manager.
- Maintain contact with the wait list clients and/or collateral service providers involved with the client.
- Maintain client records including demographic information, service agreements, assessments, service plans, correspondence, as well as regular progress notes in accordance with agency and accreditation policies and procedures.
- Completes and submits stats to admin quarterly.
