Job Description - Client Care Coordinator, Government Services
The Client Care Coordinator, Government Services is expected to perform a variety of duties that relate to client care and service delivery for our clients and their families across the community of Mississauga, Etobicoke and York Region. You will visit clients in their homes and continuously evaluate care plans through a series of ongoing communications to ensure high quality care and client satisfaction.

- Adhere to all procedures, specifically Care Consultations, Monitoring & Measurement of Service - Quality
Assurance, First Visit and Service Complaints, as mandated by our Quality Management System
- Ensure the needs of the client, family, employee, and company are consistently being met
- Work with other team members to coordinate various aspects of a client’s care
- Complete Care Consultations for new clients to EIHCC, Recharge, Surge and Nucleus and all other government programs across the city of Mississauga, Etobicoke and York Region
- Prepare care plans; enter and maintain client records for Central LHIN, Recharge, Surge and Nucleus programs in our operating system, ensuring they can be easily deciphered by Care Pros and Key Players
- Working with Enterprise Nurse Manager/RN, develop a procedure for how care plans for clients receiving Government Services are entered and updated to ensure uniformity and adherence to all franchise and municipal standards
- Develop a Quality Assurance frequency plan, performing 2-6 QA calls/visits per day
- Document and track QA visits and calls to ensure files are audit ready
- Attend EIHCC Steering Committee Meetings
- Act as a liaison with Mississauga LHIN and Central LHIN
- Create system of categorizing Government clients (levels 1-4) to support with client/Care Pro compatibility and to assist Government Services team in prioritizing staffing needs
- Work with CAREGivers and clients to identify opportunities for training; create and execute training plan
- Assist families with set up of external services (ex. appointments with OT/physio or coordination with MOW)
- Be present at trainings from OT or other SOPs and our CAREGivers when needed
- Manage client and CAREGivers complaints
- Overall tracking and updating of files to ISO standards
- Manage communication to Acclaim and monitor potential work (if they decide to resume services with HI)
- Complete all filing and uploading of paperwork for Government Services files
- Monitor current practices, identify gaps, and make recommendations to update QMS procedures
- Demonstrate open and effective communication with the General Manager, colleagues, and CAREGivers
- Share your heart by participate in the CAREGiver retention programs and coordinate CAREGiver of the month recognition
- Work independently, maintain confidentiality of information and meet deadlines
- Demonstrate consistent logic, rationality, objectivity and common sense in decision making while anticipating the consequences of decisions. Discerns when there is a need for quick decisions or more thorough assessments
- Generate new & creative approaches to problems or original modifications (innovations) to established approaches
- Assist in scheduling when required (ex. Vacation coverage, emergency time off) for the Mississauga, Etobicoke and York Region
- Host client roster reviews across the Enterprise for Government Services
- Send daily updates to GM and VP of Operations
- **Schedule is Monday to Friday 8:30-5pm**
Must have valid drivers’ license and reliable vehicle to travel from client to client Milage will be paid
- Other duties as assigned by the General Manager, Government Services
- Report directly to the General Manager, Government Services

Pay: $55,000.00-$65,000.00 per year
