Loving Hands Partners in Care is seeking a **Program Coordinator / Case Manager** with experience in **disability services** or **human services** to join our compassionate and collaborative team.
In this vital role, you’ll oversee **Individual Program Plans (IPPs)**, coordinate person-centered supports, and provide ongoing guidance to clients and frontline staff. You’ll collaborate with **People with Developmental Disabilities (PDD)**, families, and community partners to ensure quality care and meaningful outcomes.

**Key Responsibilities**
- Develop, implement, and monitor **Individual Program Plans (IPPs)** tailored to each client’s goals and support needs.
- Collaborate with **PDD**, families, guardians, and community service providers as the primary liaison.
- Maintain accurate case files, reports, and assessments in accordance with **PDD standards** and agency policies.
- Supervise, mentor, and support frontline staff to ensure consistent, high-quality service delivery.
- Partner with the Staff Coordinator to maintain appropriate client/staff ratios and program coverage.
- Maintain an active presence on the program floor to engage, observe, and foster accountability.
- Promote a **positive, inclusive, and person-centered workplace culture**.
- Maintain and update the agency webpage; develop and distribute quarterly activity calendars.
- Use digital tools such as **WhatsApp, Gmail, Google Meet, Canva, Dropbox, and Microsoft Office** for communication and documentation.
- **PDD/ISP experience** considered a strong asset.
**Qualifications**
- Diploma or degree in **Human Services, Disability Studies, Social Work**, or a related field.
- Minimum **2 years of experience** in disability services, case management, or program coordination.
- Solid understanding of **PDD standards**, person-centered planning, and community inclusion.
- Excellent communication, organization, and leadership skills.
- Proficiency with **digital communication and documentation tools**.
- Valid **Class 5 Driver’s License** and reliable vehicle.
- Current **First Aid/CPR certification**.
- **Vulnerable Sector Check** completed within the past 6 months.
