Join our team as a Social Worker where you will play a vital role in providing support and care to individuals in need. As a Social Worker, you will be responsible for assisting with patient care, data collection, case management, and offering emotional and social support to patients and their families. Your work will contribute to enhancing the well-being of our clients and ensuring they receive the necessary resources and assistance.

**Duties**:

- Provide direct support and care to patients in various settings, including hospitals and special education facilities.
- Conduct data collection and assessments to develop individualized care plans.
- Collaborate with healthcare professionals to coordinate patient care effectively.
- Offer emotional support to patients and their families during challenging times.
- Engage in case management activities to ensure the continuity of care.
- Participate in research activities related to social work practices.

**Experience**:

- Experience working with pediatric patients is preferred.
