**Job Summary**

Reporting to the Executive Director,, the support worker is expected to deliver high quality of care and service that will support patients to live comfortably and safely in their environment. The services provided will be outlined in the written plan of care prepared by the primary health care professional. Verdure Healthcare is currently recruiting for Support Workers (PSWs, RPN, RN, Caregivers) to work in long term facilities, and in homes throughout Toronto and neighbouring cities.

**Responsibilities**
- Provide personal support to clients according to the written plan of care.
- Follow Verdure healthcare and facility policies and procedures at all times.
- Assess the environment for safety during each patient visit.
- Assist clients with Rising and retiring
- Assist client with Positioning and transferring
- Dressing and undressing
- Personal grooming and hygiene
- Light meal preparation and assistance
- Treat patients with compassion and respect.
- Keep all patient information confidential.
- Work as a team with other health care professionals and service providers.

**Qualifications**
- Must be at least 18 years of age.
- Proficient verbal and written English communication skills.
- Completed Personal Support Worker certificate **OR **RN, RPN, Paramedic, Occupational Therapy, and Physiotherapy students with successful completion of Year 1 required.
- Current CPR (BCLS) and First Aid Certificate
- Related work experience within the community, in home, facility, etc with adults with physical disabilities, an asset.
- Demonstrated capability to fulfill the physical demands and time availability requirements of the position.
- A satisfactory Vulnerable Sector Check is required or VSS within the past year of hire confirming absence of a criminal history/record.
- Proof of FULL vaccination to the Covid-19 vaccine is Mandatory

**Job Types**: Part-time, Permanent

**Salary**: $22.58 per hour
- $30 per hour

Schedule:

- Short and long hour shift
- Day shift
- Evenings
- Monday to Friday
- Weekend availability

Ability to commute/relocate:Verdure Healthcare serves the GTA and neighbouring cities, you will have the option to choose your preferred location

**Salary**: $22.58-$30.00 per hour
