Support House offers supportive housing options, community outreach, peer support and building system capacity. **Diversity, Equity, and Inclusion** **The main purpose of this position** The Community Outreach Coordinator will meet people in the community who need support around housing stability, mental health, and substance use concerns. They will work alongside individuals to address urgent needs and determine longer-term goals for wellness. They will create a circle of care (Unified Care) that best meets the level of need and address OCAN needs. Services are delivered in alignment with the agency values of connection, people first, wellness and engagement through the lens of the core philosophies - trauma informed, harm reduction, housing first and safety. **Responsibilities** - Care Focused Work_ - Build rapport and establish relationships that are emotionally supportive and encourage/honoring self-determination in all interactions. Support self-agency and life skills through collaboration, role modelling, sharing information and resources. - Collaboratively document relevant information from interactions in a timely manner. - Maintain and input accurate information to client records in a timely manner. Full/Core OCAN, OCAN Self-Assessment (Peer), C-PROM (Peer), LOCUS, VISPDAT, personal profile. 2-3 business days from contact. - Provide supportive active listening and exploration to aid in a deeper understanding of the meaning of people's experiences. - Support the goals each person identifies, through offering encouragement, exploring options open to the person including pros/cons (informed choices), sharing information and education as requested/as is deemed to be meaningful. This may include collective problem solving, completing research or collaborative research with PES to explore next steps. - Collaboratively support psychological distress and navigation of crisis (understand Emergency vs Crisis vs Distress and the support required for each) - Identify strengths and achievements together. - Explore wellness tools. - Coordinate, prepare and facilitate relevant groups (life skills / house meetings/ peer self-help, peer social connection groups, Just Be You etc.) - Maintain knowledge of and collaborating with others, internally and externally/community partners for the purposes of case conferencing and care coordination, system navigation, referrals, and advocacy. - Participate in team-based services, working together to collectively to provide services. - Travel for the purpose of supporting PES and other agency requirements - As the lead-Accountable for defining client goals together, completing administration for client files, organizing client care conferences/ community supports-where you are the lead. - Work alongside individuals to provide administrative support, printing, faxing, scanning documents for the purpose of their care. - Advocate with and for PES as necessary - Collaboratively build a circle of care with the PES and team, through referrals to other supports both internally and externally - Collaboratively create transition plans with PES. Provide warm transitions to new services, through shared visits, follow-up calls etc. as needed. - Support the completion of intake/registration forms. Explain intake and registration forms. (Service Pathways Team) - Regularly share information with PES about their service rights and discuss procedures to ensure those rights (Confidentiality, Documentation, Consent, Complaint processes etc.) - Support the understanding of your role with each PES at the beginning of the relationship and throughout the relationship as needed by sharing the boundaries and limitations of your role and sharing what can be expected of your role. - Accompany PES to court, or appointments, as deemed necessary, for purpose of advocacy, literacy and emotional support. - Collaboratively support the development of various "life skills" (financial literacy, cooking, cleaning, decluttering, yard work alongside etc.) - Support PES to identify their needs related to accommodation(s) for participation in services. - Coordinate and provide access to sustainable food, personal and home products as required (groceries, furniture, hygiene supplies etc.) - Support technology literacy and coordinating accessibility to technology as needed for registration and participation in services. - Support accessibility to service through coordinati
