Urgent Response Coordinator

**Your key responsibilities will include**:
- Acting as the primary point of contact for the child/youth and family regarding URS and assessing the nature and level of need as well as strengths of the child/youth and family to inform the area of need to be supported.
- Facilitating the active participation of the family and/or the youth themselves in the planning, including goal setting and identification of service elements to address the goal.
- Providing leadership and support to the development and use of an inter-agency, inter-professional and cross-sectoral URS plan that addresses the single area of urgent need for the child/youth and family.
- Ensuring planning is collaborative and facilitating the development of a URS plan that clearly identifies the integration of the URS service elements.
- Identifying and coordinating consultation with specialized services to address the area of urgent need including but not limited to arranging necessary assessments for the child/youth.
- Monitoring and reviewing the URS plan and delivery of service on a regular basis and planning for transition.
- Working collaboratively with Project Partners in the delivery of the URS Ontario Autism Program (OAP) including participation on the admission team and other committees as required.
- Researching and providing information and resource materials to service providers as necessary and appropriate to increase their understanding of and access to services for children, youth and families.
- Assisting service providers in the development of overall service plans, strategies and activities based on the priorities of the family as requested.
- Providing assistance to primary workers to support the implementation of individual plans as requested.
- Maintaining program administration including client record keeping, documentation of data required by the Ministry of Community, Children and Social Services (MCCSS) and Resources for Exceptional Children and Youth and implementation of evaluation strategies to assess the service impact.
**What You Bring**:
- Bachelor’s degree in social work.
- Licensed/registered member of the College of Social Work in current good standing with expectation to maintain good standing.
- 5 years clinical experience related to children and families and demonstrated expertise in clinical practice relevant to the identified program area.
- Strong counselling skills: individual, family, group, crisis intervention, conflict resolution, and grief/palliative.
- Experience working with families of children with ASD is preferred.
- Commitment to family-centred and solution focused coaching approaches.
- Demonstrated ability to work independently and to provide service in a flexible, comprehensive manner.
- Demonstrated commitment to protecting privacy and client confidentiality.
- Excellent written and verbal communication skills, including experience with written reports for clients and external agencies and active listening skills.
- Familiarity with governing legislation and supporting compliance by parents and service providers.
- Knowledge of community resources and children’s service system.
- Available to work the occasional evening as required.
- Access to reliable and efficient transportation to travel regularly to a variety of different locations.
- Clear vulnerable sector check and updated immunizations including proof of COVID-19 vaccination (two doses) required.
