Description:
The Area Manager, Transition Services Peter Lougheed Centre (PLC) and Personal Care Homes (PCH) reports directly to the Director, Transition Services Calgary Zone and plays a key leadership role within the programs. The position is responsible and accountable for providing direction, leadership, and organization over the activities of the Transition Services teams at PLC and PCH. The position is also responsible and accountable for established capacity management strategies that support acute care through to the community, and safe patient transitions from acute care to Integrated Home Care and Continuing Care Homes. The Area Manager works in alignment with the AHS vision, mission and values and Transition Services program priorities and business plan. The Area Manager makes key decisions about the allocation and integration of Transition Services and PCH resources to carry out these activities and actively seeks to develop and maintain effective working relationships with key AHS and external stakeholders.

**Required Qualifications**:
A minimum of a bachelor's degree in a healthcare field is required, related field is preferred. A minimum of 3 years healthcare management experience required, Seniors Palliative and Continuing Care experience preferred. Active registration with respective professional college.

A demonstrated pattern of professional and personal development. In addition, the successful applicant will also possess a record of accomplishment in the following areas: Demonstrated knowledge of community agencies and resources to facilitate the linkage to appropriate services. Knowledge of seniors' health and life transitions and a working knowledge of health care system. Demonstrated ability to effect change and create innovative solutions for complex and diverse issues. Client-focused orientation and strong commitment to quality team-based service delivery. Progressive leadership experience including coaching and teaching to inspire and build confidence in others. Excellent analytical skills and demonstrated ability and comfort with decision-making responsibilities. Effective communication with strong organizational & influential skills. Ability to manage human, financial and physical resources within an operating environment. Ability to lead organizational change and create innovative solutions for complex and diverse issues. Ability to establish and maintain partnerships with internal and external stakeholders toward the achievement of organizational goals.
