Counsellor - Community Support Worker

**Job Summary**
We are seeking a Counsellor/Social Worker who has some experience in hospice, palliative care and bereavement to help support a maternity leave of absence, working two days per week.

**Duties**
- Complete intake services for virtual, in-office and in-home visiting for palliative care, caregivers, and bereaved.
- Manage case load of community clients for both group support and one-on-one counselling.
- Work collaboratively with external referral agencies and refer to other healthcare professionals and community agencies.
- Provide timely and comprehensive information to the Wish & Volunteer Coordinator, and the Community Support Worker - Programs & Volunteers on matching volunteer skills for client/family needs.
- Assist with the development and coordination of programs and services working closely with Community Support Lead, Lead Therapist, other Community Support Workers, Coordinator of Volunteers and trained Volunteers.
- Assist with special events such as an annual memorial service.
- Provide support and guidance for placement students as part of a team approach to their program needs.
- Attend bi-weekly team Clinical Supervision meetings, and bi-weekly Community Team meetings.
- May be required to work after normal working hours for assessment purposes, volunteer training, community education or special events. Flexibility to have a mix of working from home and in-office.

**Experience**
- Bachelor of Social Work or Counselling Degree preferred, and in good standing with a college or governing/regulatory body.
- 3-5 years' experience preferred.
- Experience in case management, care planning and program development required.
- Working directly with vulnerable clients required.
- Strong communication skills and the ability to build rapport with clients and their families.
- Knowledge of community resources and willingness to learn more.
- Strong organizational and time management skills.
- A positive demeanor with the ability to deal with change and be innovative.
- Use of a personal vehicle is required.

Contract length: 12 months

Pay: $30.00-$35.00 per hour

Expected hours: 16 per week
