Sinai Health is looking for a Clinical Practice Leader to support our Pharmacy Department. The role is primarily located at our Mount Sinai Hospital campus.

Reporting to the Senior Manager, Clinical Pharmacy, the Clinical Practice Leader will actively demonstrate, encourage and maintain effective clinical services in pharmacy and provides clinical leadership to staff pharmacists through mentorship, teaching, and research. This person will maintain a clinical practice (80% of time), carry clinical leadership, learner education, and practice-based research responsibilities (20% of time, combined). These includes modelling excellence in clinical practice, leading and overseeing clinical practice changes, and leading and supporting quality improvement projects tied to pharmacists’ clinical practice.

**About the Pharmacy Department**

Our Pharmacy Team at Mount Sinai Hospital ensures that medication management is handled with the utmost care and precision. Our services include medication dispensing, inventory control and compounding medications. We support patients by ensuring they receive accurate and timely medication administration, which is vital for their recovery and treatment efficacy. The team also assists health care providers and collaborates with various departments to streamline the complex process of medication distribution. Through our inventory management and ongoing quality assurance activities, we uphold the highest standards of patient care, reflecting our dedication to the health and well-being of our community.

**In this role you will**:

- Support and maintain the philosophy, goals, objectives and standards of the department of Pharmacy
- Oversee drug therapy management of patients in assigned area:

- Maintain a clinical practice area (80% of time)
- Support and promote patient focused pharmacotherapeutic service, including medication assessment, recommendations, monitoring, counseling and education
- Participate in patient and multidisciplinary rounds
- Undertake consistent documentation in patient charts, pharmacy computer system and pharmacy documentation software
- Develop effective working relationships with physicians and other health care professionals to ensure safe, effective, and cost-effective drug therapy
- Serve as a clinical resource and specialist and leader in terms of knowledge and clinical skills for pharmacists, physicians, other healthcare professionals and students
- Maintain an environment conducive to positive staff morale and workplace satisfaction
- Promote and support collaboration and teamwork among pharmacy staff
- Provide operational support to the pharmacy with regards to order verification, clinical coverage as determined by need. Operational support is included as limited shifts in the pharmacy dispensary
- Represent the pharmacy department on internal and hospital-wide committees and actively contributes to accomplishing the goals of the committee or subcommittee.
- Participate in Pharmacy Leadership activities and meetings
- Encourage pharmacist participation in policy and procedure development, and quality improvement work to advance the quality of patient care
- Lead the implementation of best practices and standards, informed by Accreditation Canada, the Ontario College of Pharmacists, and the Canadian Society of Hospital Pharmacists
- Teach students and peers in pharmacy, medicine, and allied health. Precept pharmacy learners of all levels
- Assist with the planning and facilitation of education and continuing education strategies within the department of pharmacy
- Assist staff pharmacists with projects and project presentations at conferences and/or writing for medical journals
- Participate in and/or generate patient oriented and quality of care projects and research within the pharmacy department or in collaboration with other health care professionals to enhance patient care and pharmacotherapeutic outcomes
- Complete annual performance plans of staff pharmacists
- Assist in recruitment and selection of clinical pharmacists, residents and/or students
- Participate in planning and development of pharmacists schedule, including day-to-day adjustments to clinical coverage as required
- Identify, plan, and facilitate pharmacist training and development based on individual goals and organizational needs
- Perform other duties consistent with the job classification as assigned
- Job Requirements

**Job Requirements**:
Mandatory
- Successful completion of a degree or diploma in a relevant discipline from an accredited educational institution
- Licensure with the Ontario College of Pharmacists
- Minimum of 5 or more years of experience as a hospital-based clinical pharmacist
- Minimum of 2 or more years of experience directly monitoring pharmacy learners, including pharmacy residents
