Youth Mental Health Case Worker

**Scope**:
Under the direct supervision of the Youth Lodge Program manager and the overall supervision of the Executive Director, the purpose of the Indigenous Youth Mental Health Case Worker is to assist youth who are living with poor mental health and attain a better quality of life (Mino B’maadzawin). The Youth Mental Health Case Worker will be assisting with the coordination of integrated services delivery from referral, intake, discharge and on-going aftercare for Indigenous youth in all of the identified service areas.
**Responsibilities**:
- Provide direct services to a minimum of 50 youth who are at risk or high risk.
- Participate in residential youth programs, as scheduled.
- Provide direct care to Indigenous youth through counselling, referrals, accessing assessments and group work.
- Developing plans of service (care) in collaboration with the youth to implement the plans of service.
- Coordinate aftercare for youth who have attended programming.
- Coordinating and facilitating access to traditional healing and other cultural programming.
- Participating in treatment case conferences, in order to facilitate integrated service delivery.
- Making recommendations to the youth regarding alternative services to assist with linking to other community resources using a client centred approach.
**Qualifications**:
- A Bachelors Social Work degree preferred or an undergraduate degree in a related field with experience working within the Indigenous community.
- A minimum two-year Social Service Worker diploma in a related field of addictions, mental health, social issues with three years’ minimum experience of direct counselling, crisis intervention and service to Indigenous youth.
- Willing to obtain CPR/First Aid, NVCI and other trainings as required.
- Must be able to provide a clear Vulnerable Sector Check.
- Must have a valid drivers license and reliable transportation.
- Must be a role model in the community.
**Job Types**: Full-time, Permanent
Pay: $50,000.00-$55,000.00 per year
