Pursuit Health Management is a multi-disciplinary rehabilitation firm located in London, Ontario, providing services throughout Southwestern Ontario for the past 26 years. Our healthcare professionals include Occupational Therapists, Case Managers, Social Workers, Rehab Therapists and Kinesiologists.

**Case Manager**:
A creative problem solver with a customer focus and a patient-friendly approach, you will provide case management services to our clients. Utilizing proven communication and negotiation skills, you will be an integral member of a multi-disciplinary team offering unparalleled client centered services.

**Responsibilities**:
Pursuit Health Management clients have been injured in motor vehicle accidents, suffered a workplace injury or are in receipt of disability benefits. This position requires coordination of rehabilitation services for injured/ill individuals to assist them in achieving maximum medical/functional recovery.

On-site visits to a hospital, the client’s home, physician's office and/or workplaces are required. The Case Manager communicates regularly with referral sources (insurers, lawyers, other healthcare professionals) and provides timely written reports, according to established standards. Clients can range in age from pediatric to geriatric

Assisting with multi-system navigation on behalf of the client is an integral component of the position.

**Skills**:

- Strong assessment and planning capabilities to ensure client progression along their recovery path
- Superior communication skills
- Demonstrated interviewing, medical assessment and disability analysis skills
- Experience consulting with members of the healthcare team (physicians, occupational therapists, physiotherapists, psychologists, Speech Language, etc.) and the legal community
- Ability to provide comprehensive reports reflecting client needs within a treatment plan framework
- Experience with managing a varied caseload involving a variety of impairments and meeting required timelines
- Experience with neurological, cognitive (acquired brain injury) and orthopedic impairments, as well as the ability to manage complex psycho-social conditions and identify appropriate treatment interventions to enhance client function
- Extremely organized with the ability to multi-task
- Excellent computer/ typing skills mandatory
- Highly developed customer service abilities
- Superior knowledge of community based resources
- A vehicle is required as travel throughout Southwestern Ontario is a component of the position.

**Qualifications**:

- University (Nursing, Social Work or Bachelors Degree ) or Nursing Diploma
- 3+ years experience in case management of complex cases is an asset

**Job Types**: Part-time, Permanent

Expected hours: 24 - 30 per week
