Reporting to the Clinical Manager - Geriatric Assessment & Rehabilitative Care at SJH with a matrix reporting to the Manager, Emergency Department at TBRHSC, the Geriatric Emergency Management (GEM) Nurse is responsible for providing specialized nursing services in the Emergency Department (ED) of TBRHSC for older adults living with/at risk of frailty.

The GEM Nurse provides expert and targeted geriatric assessment and treatment services in the ED; enhances linkages with hospital and community services; advocates for the older adults living with/at risk of frailty; and makes recommendations and referrals directly to community, outpatient and inpatient geriatric or older adult programming as required. The GEM Nurse will work alongside interprofesional colleagues in the ED and supports the overall goals and objectives of the Frailty Identification and Transition project.

THE GEM Nurse is responsible for helping support transitions in care for older adults with a focus on early identification and prevention of frailty, restoration and rehabilitation, and health teaching with the intent of increasing the patient’s ability to stay safe and well in the community and thus reduce ED visits, hospitalization, ALC bed utilization and delay admission to Long-Term Care. The GEM Nurse works collaboratively with the patient Flow Coordinator at TBRHSC and the Client Flow Coordinator at SJHOSP to support client transitions and will also contribute to capacity building among others working in the ED, as well as community partners, by sharing knowledge and skills, acting as a resource and collaborating with others in the care of older adults. The GEM nurse is able to spend time mentoring and coaching, and supporting the implementation of Senior Friendly Care and geriatric protocols in the ED.

Key responsibilities include early identification, screening, assessing, navigating, and supporting patients eligible for Community Support Services, Home and Community Support Services, Outpatient Specialized Geriatric Services and the Geriatric Assessment and Rehabilitative Care program including Remote Client Monitoring (RCM) programming with direct referral.

**Qualifications**:
Education/Experience:

- Current unrestricted certificate of registration with the Ontario College of Nurses
- Current BLS certification
- Minimum of five (5) years’ experience working within Specialized Geriatric Services and/or with geriatric population
- Completing assessment and treatment guided by the Comprehensive Geriatric Assessment.
- Advanced knowledge, skills, understanding of best practices related to dementia, delirium, mental health and its effect on the person and family members/care partners

Skills/Abilities:

- Excellent critical thinking, interpersonal, diplomatic and communication skills (verbal and written)
- Effective listening, observation and facilitation skills to effectively assess the client and provide appropriate treatment and consultation
- Ability to function and contribute as an effective team member and to work independently
- Excellent problem recognition skills with the ability to proactively achieve resolutions both through direction and negotiation and make decisions rapidly
- Demonstrated knowledge, understanding, and commitment to client-centred care
- Experience in providing focused geriatric assessments using various tools in order to identify high risk geriatric patients
- Knowledge of community and regional services/resources for the geriatric population
- Demonstrated leadership experience and collaborative leadership style
- Demonstrated discharge planning experience
- Established networks with community/regional partners who specialize in servicing geriatric populations
- Demonstrated adherence to PFCC philosophy, knowledge of CNO standards of practice
- Demonstrated leadership and participation in patient flow initiatives
- Demonstrated ability to work under pressure and manage competing work demands
- Proficient computer skills in Meditech, Strata, Excel, Word & Power Point programs
- Demonstrated regular attendance at work

Assets:

- CNA Certificate in Geriatric Nursing/Rehab Nursing
- Membership in professional association e.g. RNAO
- Experience working with Indigenous persons and First Nations
- Knowledge of multi-cultural beliefs, values and perspectives with particular emphasis on Indigenous persons and First Nations

**CONDITION OF EMPLOYMENT**- Ability to work irregular hours
- Must undergo a successful Police Vulnerable Sector Check (PVSC)
- Valid Ontario driver's License
