Transformation Specialist

The Transformation Specialist will be a key member of the Transformation Office, reporting to the Director, Transformation and Partnerships. The role will focus on supporting the development of multiple Ontario Health Teams (OHTs) that Unity Health Toronto is involved in. This work will support greater integration of services and operations to improve patient outcomes and experience, provider experience and overall efficiency.

The Transformation Specialist will lead the planning of multiple work streams for OHTs. She/he will do this using her/his in-depth health system knowledge, ability to work and plan in a complex and ambiguous environment, and her/his strong sense for change management. This role will also require working in close collaboration with external partners, drawing on robust stakeholder management skills. The Transformation Specialist will have a solid understanding of leading models of system integration, stay abreast of Ministry of Health information and direction, and will leverage learnings from other developing Ontario Health Teams. She or he will support analysis of data to understand the needs of the populations of the OHTs and inform planning. She or he will use critical thinking skills to assess different elements of the proposed approaches of the OHT considering the outcomes for patients, providers, the system and Unity Health Toronto.

The Transformation Specialist will provide comprehensive project management to facilitate the implementation of the Ontario Health Teams. He or she will do so by developing and executing implementation plans for these large scale projects, considering inter-dependencies both within and across OHTs. The Transformation Specialist will use his/her excellent communication skills to keep all stakeholders updated and work progressing and aligned. Fostering and developing strong relationships with partners will be essential to successful implement Ontario Health Teams. The Transformation Specialists will also support the identification and assessment of new partnerships.

**Qualifications**:

- Eight (8) to Ten (10) or more years project management experience in a health care environment;
- University/College Degree in Business Management or Health Management or equivalent to years of relevant experience;
- PMP Certification preferred
- Demonstrated experience in facilitation, collaboration and negotiation with multiple stakeholders/service providers;
- Solid understanding of provincial health care and multiple sectors;
- Strong analytical skills to assess, interpret and recommend actions based on the needs of each OHT’s population
- Excellent verbal and written communication skills with the ability to communicate information/ideas, in a manner that is easily understood by others;
- Excellent computer skills e.g., Microsoft Word, Excel and Powerpoint, required;
- Excellent presentation and negotiation skills with the ability to influence others;
- Ability to deal with situations where information is difficult to obtain, complex or ambiguous;
- Understands operational excellence, quality improvement, change management and process redesign concepts including lean/six sigma;
- Ability to work in a fast pace environment and motivate project staff and others to achieve a common goal;
- Ability to interact with people from all disciplines/levels, be flexible/adaptable to change and maintain confidentiality;
- Ability to work well independently and collaboratively as a team player with a variety of staff/stakeholders to undertake detailed analysis, report generation and project implementation;
- Demonstrated team player with the ability to be flexible and adaptable to change;
- Satisfactory attendance record
