Moira Place, part of the AON group of homes, is a vibrant and innovative long-term care community located in beautiful Tweed, Ontario. We are currently seeking a **Clinical Assessment Coordinator** to join our leadership team and support excellence in resident care through strong clinical assessment practices and team education.
**About the Role**:
Reporting to the Director of Care and working closely with the RAI Coordinator, the Clinical Assessment Coordinator supports the interdisciplinary team in completing timely and accurate interRAI-LTCF assessments. This role is vital to optimizing funding, guiding quality improvement, and fostering a culture of learning and best practices in care delivery.
**Key Responsibilities**:
- Assist in the completion of interRAI-LTCF assessments and ensure compliance with timelines
- Provide education and one-on-one coaching to clinical staff on documentation and assessment requirements
- Support the Restorative Care program in collaboration with the Restorative Care Aide and other team members
- Audit care plans and documentation, identify areas for improvement, and support action plans
- Contribute to staff training, performance support, and policy compliance
- Participate in continuous quality improvement initiatives and data reporting
**What We’re Looking For**:
- Current registration with the **College of Nurses of Ontario (CNO)**:
- **1-3 years of experience** in a long-term care setting (preferred)
- In-depth understanding of **interRAI-LTCF assessments** and clinical documentation standards
- Excellent communication, coaching, and adult education skills
- Strong organizational and time management abilities
- Knowledge of **FLTCA regulations** and **MLTC funding mechanisms**:
- Proficient in clinical software and digital documentation tools
