Community Paramedic

**The start date will follow the selection process.**

**Characteristic Duties**:Under the general supervision of the Commander of Community Paramedicine.

1. Develop, implement, monitor and report results for health promotion, education, injury prevention programs to assist citizens in chronic disease recognition, prevention, referral and health system navigation process, and public education and injury awareness/prevention strategies.
2. Develop and implement Community Paramedic Program policies, collaboration agreements, and training programs.
3. Develop “canned programs” with all associated processes, polices, training, documentation for contacts for each component of the program.
4. Provide health care assessments to identified patient populations in support of the Community Paramedic (CP) Program.
5. Perform patient assessments and interventions in accordance with approved CP Medical Directives, processes, procedures, policies, and standards.
6. Coordinate and facilitate the delivery of education, quality programs and information for patients, and the public on services provided, as required.
7. Liaise and communicate with patients, family, physicians, community partners and other healthcare stakeholders for the provision of CP care to reduce patient requirements for hospital services.
8. Complete administrative documentation specified by the CP program.
9. Inspect program vehicle and all equipment at the start and throughout the shift to ensure they are in proper working order and in a clean and sanitary condition. Check and ensure the vehicle is stocked at all times for operational readiness.
10. Participate in research development and evaluation of medical devices, practices, and procedures.
11. Participate in mandatory training programs, continuing medical education programs, quality assurance programs and divisional training programs, as required.
12. Participate in and prepare reports on program demographics, opportunities, trends, and outcomes, and makes recommendations for program improvement.
13. Provide mentorship and training of community paramedics.
14. Participate in research initiatives involving Paramedic Services.
15. Deliver public education sessions to different groups and forums.
16. Ensure good public relations standards are maintained with patients, family, physicians, healthcare team members, public, and other government officials or outside organizations.
17. Respond and function as a Primary Care Paramedic as required in accordance with the Primary Care Paramedic job description.
18. Participate on committees, work groups, task forces and special projects, as assigned.
19. Manage information in accordance with legislation and corporate standards.
20. Develop and maintain a thorough working knowledge of CGS’s Safety Manual and the applicable Provincial Legislation listed herein.
21. Performs other related duties as required.

**Qualifications**:

- Successful completion of an accredited Primary Paramedic Program.
- Current PCP certification with a recognized Base Hospital Program to be maintained.
- Professional development in the field of Community Paramedicine or other health related filed considered an asset.
- Degree in health-related field considered an asset.
- Minimum 2 years’ experience as a Primary Care Paramedic.
- Experience as a Community Paramedic considered an asset.
- Previous experience developing and facilitating health related adult education considered an asset.
- Knowledge of and demonstrated ability to incorporate core competencies including customer service, communication, teamwork, initiative/self-management, accountability, and flexibility/adaptability.
- Strong communication skills and the ability to foster a positive and cooperative working relationship with aged and vulnerable populations, team members, allied services, other agencies, and the public.
- Ability to meet program objectives and deliverables including meeting established milestones and timelines.
- Able to work independently with mínimal supervision.
- Ability to work shift work and outside regular EMS shifts assignments/patterns including rotating shifts, evenings/nights, weekends, and statutory holidays.
- Knowledge of CGS’s Emergency Services structure and organization.
- General knowledge and demonstrated ability in instructional design, analysis measurement and evaluation methods.
- Best practices within areas of responsibility.
- Mental and physical fitness to perform essential job functions.
- Excellent use of English; verbally and in writing.
- French verbal skills an asset.
