- **JOB SUMMARY**:The Registered Social Worker works within a multi-disciplinary team to provide wholistic professional mental health prevention, assessment, referral, clinical counselling, consulting and support services to individuals and family members who face a range of complex mental health needs. Work involves comprehensive assessments and education for individuals, families and groups to determine and implement in-depth and wholistic treatment, prevention, and therapeutic intervention plans, as well as work with communities to identify, educate, and respond to mental health needs.
- Develop, promote and support culturally based activities, programs and engagement events that impact balance and wellness for community members, as well as evaluate program effectiveness
- Prepare action plans and budget submissions, in advance, for community events or workshops being organized and submit them to the manager for approval
- Track early identification of mental illness and disorder
- Provide early intervention strategies, support and advocacy by engaging people with mental illness in community participation, prevention of relapse and promotion of recovery through programs such as: residential rehabilitation, work in clinical settings, home based outreach and centre-based programs delivered by community based non-government organizations
- Screen referrals for mental health and addictions services; utilize determinants of crisis interventions as needed; maintain wait lists; prioritizes cases; and complete intake assessments
- Complete comprehensive assessments of mental health and/or addictions clients through in
- depth clinical interviews
- Formulate therapeutic interventions/treatment plans and follow up as required
- Utilize evidence-based techniques delivered within an Indigenous framework
- Provides therapy, counselling, and follow-up to clients, and modifies case plans as treatment progresses
- Completes thorough case notes and documentation in an electronic medical record

- Collaborate with the community to identify and obtain resources and/or services which involves facilitating focus groups, and providing education and training sessions in the areas of mental health and addictions
- Ensure the necessary collaboration and case conferencing with other relevant staff and agencies in the provision of services to clients in accordance with the Mental Health Act
- Serve on organizational and external committees, and focus/working groups to ensure that clients are receiving support, advocacy, referral and access to services
- Complete clinical reports and assessments as requested for various outside agencies, IIPCT, and court and testify in court as required
- Ensure all operations are conducted in a safe manner and in accordance with IIPCT Policies,
Canada Labour Code Part II and Occupational Health & Safety Regulations
**EDUCATION, EXPERIENCE, KNOWLEDGE, SKILLS AND ABILITIES**
**REQUIREMENTS**:
- Master of Social Work (MSW) (Will consider a Bachelor of Social Work (BSW) or Diploma in Social Service Work with experience)
- Must be registered with the College of Social Workers and Social Service Workers
(OCSWSSW)
- Experience working with Indigenous populations in a mental health setting
- Knowledge of Mental Health and Addictions related treatment, policies, procedures and trends, as well as theories and assessment
- Specialized clinical knowledge of individual, family and group therapy, counseling, crisis management, and advocacy services
- Knowledge of research and evidence-based practices, educational promotion and prevention and as well as community culture and resources
- Knowledge of Indigenous culture, traditions, ceremony and connecting clients to cultural practices that can be used in supporting clients.
- Awareness of the recent, historical, and intergenerational trauma that Indigenous individuals and their families may have experienced, including the psychological, neurological, physiological, social and spiritual impact of trauma; and
- Strong mediation, negotiation skills, emergency response, and advocacy skills
- Resourceful, critical thinker with problem solving skills, as well as proven skills in organization and multi-tasking
- Ability to use tools to assess individuals for Mental Health and Addictions and develop plans that meet client needs
