The Health Services Caseworker is responsible for the provision of case management services with a focus on health care, including harm reduction and mental health supports. The Health Services Caseworker will foster relationships with residents, develop and implement health promotion programming, and connect with external health care partners to refer residents as needed. This role will collaborate with other members of the casework team to offer a well-rounded support to both current and former residents. We are committed to serving people who face barriers to health care and the Health Services Caseworker will focus on reducing these barriers.
**RESPONSIBILITES**
- Communicate effectively and use a professional approach with all team members, supervisors, community members, and other stakeholders;
- Collaborate with team members to enhance programs and services and troubleshoot issues;
- Identify barriers and provide suggestions to improve accessibility;
- Engage in supportive communication and conflict resolution;
- Maintain positive relationships with other service providers, including but not limited to community partners, contractors, and emergency personnel;
- Keep professional documentation and records as required;
- Coordinate on-site health services with a focus on harm reduction;
- Provide individualized case management, coordination of services, appointment support, referrals, and advocacy as needed on relevant health issues;
- Connect clients with support in the community to help them achieve and maintain stability, including support groups, life skills groups, and social events;
- Complete and compile all necessary file notes, reports, statistics and other written documentation on a timely basis and in accordance with agency standards;
- Collaborate with case workers and other members of the transitional house team in coordination of support for residents;
- Develop individualized plans of action with short
- and long-term goals and identifying strategies to achieve those goals;
- Enhance the capacity of internal and external stakeholders by providing timely feedback and coaching to support harm reduction and health promotion engagement;
- Model a trauma-informed, anti-oppressive approach to health care and harm reduction practices;
- Develop and maintain positive and collaborative partnerships with community partners;
- Participate in the development, implementation and evaluation of services provided;
- Contribute to the strategic and operational planning process;
- Provide or support the development of educational and awareness sessions, briefings, and resources pertaining to the health needs of community members;
- Attend and engage in staff meetings and training sessions;
- Follow all operational protocols, including incident and injury reporting;
- Keep informed about and comply with all updates to Health and Safety best practices including IPAC and PPE requirements;
- Direct questions and concerns to the appropriate departments;
- Exercise time management and task prioritization skills;
- Attend scheduled shifts;
- Other duties as assigned.

**WORKING CONDITIONS**:
- Interaction with employees, management staff, and the public at large;
- Interaction with people under varying circumstances including situations of a highly sensitive nature;
- Interaction with guests, residents, and other community members with complex needs related to poverty, trauma, health issues, and food and housing insecurity;
- High levels of trauma content and pressure;
- Flexible hours, including nights, weekends, and holidays;
- Intermittent physical activity including walking, standing, sitting, lifting;
- Safety precautions due to exposure to pets, pests, hazardous waste, and contagious illnesses;
- Travel between locations and some scheduled overtime is associated with this position
**QUALIFICATIONS**:
- BSW, SSW, Addictions/Mental Health, Health Promotion or another health-related field of study or demonstrated experience in a related field
- Crisis Prevention and Intervention certificate
- CPR, First aid certificate
**EXPERIENCE**:
- Minimum 1 years' experience working with diverse populations including people experiencing mental health, substance use issues, histories of trauma and homelessness;
- Case management documentation and case management system navigation;
- One-on-one counseling;
- Facilitating partnerships with external agencies;
- Workshop and training facilitation;
- Lived experience considered an asset.
