Our 55 offices in Canada offer a wide range of exceptional in-home care services, such as companionship, outings, personal care, nursing care and hospice-at-home. We are looking for our next new addition in the unique role of Community Care Planner and Community Liaison. This role works with families in the community, understanding care options, completing client assessments, building Care Plans and hiring, developing and nurturing field staff. This role also supports community engagement, local brand awareness, business development in the **Halifax Region**. Must have a valid driver’s license and access to a personal vehicle. Must be available for an on-call rotation of approximately 1 weekend per month. References required.

**Responsibilities as Care Planner and Community Liaison**
- Conducting client assessments and developing Care Plans, including in-home assessments in the Halifax region
- Acting as a resource to client families
- Spearheading the Care Team assembly
- Prioritizing client and caregiving staff satisfaction
- Participate in the initial screening, hiring and supervision of caregiving staff
- Supervising, managing and motivating the Care Team members
- Relationship development with caregivers, clients and their families.
- Drop off PPE when needed
- Engage directly with the Right at Home management team, business development representatives, and other staff as an integral part of the company’s goal to continue to be Canada’s leader in the delivery of family-funded home health care services
- Leading CCA/PSW team in collaboration with Care Coordinator.
- Supervision and support of the care team and problem solving with clients and families.
- Schedule and conduct meetings with various referral sources in the community to develop business opportunities, expand footprint within designated territories
- Conduct exceptional follow-up to your referral source meetings and meaningfully grow those relationships.
- Assist in achieving an exceptional experience for the Caregivers within our Right at Home system through warm, positive communication, encouragement, inclusiveness and leadership.
- Other administrative duties as assigned, as well as office upkeep and filing
- Be an excellent brand ambassador for the Right at Home

**Essential Functions - Direct and Delegated Responsibilities**
- Travel across the Halifax region is an expected and anticipated part of your role. Conducting meetings, consultations, and engagements will happen anywhere within the broad region.
- Drive some marketing strategy, social media strategy, business development, business growth and other communication channel strategy for brand and services promotion.
- Schedule and conduct meetings with various referral sources to provide awareness of, and promote, Right at Home supportive care services, based on the classroom and field training.
- Conduct exceptional follow-up to your referral source meetings and meaningfully grow those relationships.
- Monitor and attend relevant community events for community-awareness opportunities.
