**Patient Care Manager Transition Management** - Status:_ Regular Full Time - Department:_ Transition Management - Program:_ Healthy Aging 8 Hour Shifts, Days Schedule (Subject to Change) - Location:_ Oshawa - Wage Schedule:_ $55.120 - $77.830 per hour - File Number:_ 2400003930 **DUTIES AND RESPONSIBILITIES**: Reporting to the Director, Healthy Aging, the Patient Care Manager (PCM), Transition Management is responsible for providing oversight of the transition processes throughout the organization. The PCM, Transition Management, provides leadership through developing, implementing, monitoring and evaluating systems to create seamless transitions. Providing leadership to ensure optimization of resources within inpatient areas and supporting the pathways to transition to the community. Partner closely with internal/external stakeholders for the delivery of outcomes, communication of complex concepts, building consensus and supporting change management to ensure best practices for transitions are enabled. The PCM, Transition Management, will further support initiatives within the community, providing visible leadership to ensure optimal performance management and smooth operation of these service areas. The role will provide expert problem management support to resolve complex implementation issues: conducting root-cause analysis, seeking out linkages and developing pathways for transitions. The role will skillfully manages daily operations of teams supporting transitions across the organization providing consistent messaging, guidance and problem solving. **QUALIFICATIONS**: "Undergraduate degree in Health or Business related discipline (required); "Graduate degree in Health or Business related discipline (preferred); "Two years recent clinical/leadership experience (required); "Demonstrated knowledge of community supports and awareness of current challenges within the catchment area; "Demonstrated understanding of change management; "Experience within community an asset; "A tactical thinker with experience in developing policies and protocols; "Minimum of two (2) years' experience in quality improvement development, implementation and evaluation; "Project Management skills and experience; "Related knowledge in related clinical specialty; "Knowledgeable in the principles of adult education and teaching/learning principles; "Knowledge of current trends in nursing, allied health and health care as well as applicable legislative and hospital/program policies; "Demonstrated knowledge of quality improvement principles, methods, and tools with excellent project management skills including assessment, planning, implementation and evaluation skills at a program and corporate level; "Strong analytical skills including the ability to collect, analyze, share and interpret qualitative and quantitative data; "Effective communication (written and verbal) and interpersonal skills; "Demonstrated ability to lead effectively and create a high performance culture that facilitates growth and development. **CONDITIONS OF EMPLOYMENT**: The terms and conditions of employment are in accordance with Lakeridge Health's policies and procedures.
