**KEY FUNCTION**:
Provide comprehensive social work services, including counselling and advocacy, to clients struggling with a mental health and addiction illness, and promote a successful transition from homelessness to permanent housing.

**REPORTING**:
Under the general direction of the Clinical Manager, Lotus Program.

**DUTIES**:

- Provide clinical services to address the specific psychosocial needs of clients in the transitional housing program.
- Conduct in-depth psychosocial assessments of client’s needs, strengths, and challenges.
- Maintain up-to-date charting/records in compliance with HSN, departmental policies and procedures, and the Ontario College of Social Workers and Social Service Workers standards of practice.
- Provide therapeutic clinical intervention and education for individuals, and families and groups, as required, addressing the needs of individuals, including therapy and emotional support.
- Act as advocate for available resources necessary for care and basic living needs.
- Provide individual and group counselling on issues related to trauma, emotional and mental health challenges, and substance use disorders, including harm reduction models of care.
- Advocate and implement interventions, service deliveries and policies that encourage and support diversity (e.g. gender, language, sexual identity, cultural, etc.).
- Lead the implementation of quality improvement initiatives and program evaluations, while collaborating with HSNRI scientists on the current research component of the program.
- Consult with appropriate team members as required.
- Receive and provide peer supervision.
- Supervise and evaluate university level clinical social work practicums.
- Organize and participate in staff meetings, clinical rounds, conferences, workshops and in-services within HSN and in the community.
- Maintain relevant statistical information.
- Develop and implement clinical procedures and standards of work.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Educate and promote health, safety and wellness in the work place.
- Represent the department or program on various committees and in meetings as required.
- Perform other duties as required.

**QUALIFICATIONS**

**EDUCATION AND TRAINING**:

- Minimum of a Master's Degree in Social Work from an accredited university.
- Current Certificate of Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is required.
- Certification in Cognitive Behavioural Therapies (CBT), Family Interventions Therapy (FIT), Motivational Interviewing (MI), and Mindfulness-based stress reduction (MBSR) is preferred.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

**EXPERIENCE**:

- Minimum of three (3) years’ experience in mental health and addiction.
- Minimum of five (5) years of related practical experience working within a healthcare environment.

**KNOWLEDGE/SKILLS/ABILITIES**:

- Demonstrated knowledge of government legislation, policies and programs affecting the patient population served (i.e. Health Care Consent Act/Mental Health Act and Child Protection).
- Demonstrated working knowledge of general adult mental health issues, crisis intervention, and impact of MI and Addiction trajectory on these issues.
- Demonstrated comprehensive knowledge of community resources and eligibility criteria.
- Demonstrated knowledge of ethical and psychosocial issues related to mental health and addiction.
- Demonstrated strong understanding and sensitivity to family and caregiver support issues.
- Demonstrated skills in assessment and intervention with individuals, groups and families.
- Demonstrated understanding and experience of family systems and dynamics.
- Demonstrated ability to provide group-based interventions, both psychoeducational and therapeutic.
- Demonstrated knowledge of mobilizing community health resources and enhancing continuum of care.
- Demonstrated knowledge of Indigenous culture.
- Demonstrated ability to effectively problem-solve, set priorities, and manage conflict.
