**Salary**: $52,200-$61,800
**Position Description**: Reporting to the Program Supervisor, as the Registered Social Worker, you will be working within the context of a collaborative interdisciplinary team reflecting the principles of person-centered and evidence-based care by assisting with coordinating and delivering mental health and addiction services to tenants. You will be supporting these individuals who have experienced long term housing instability and/or long stays in hospitals or in other institutional settings. This position will require a high level of resilience and a willingness to work near challenging situations.

**Hours of Work**: Full time 40 hours per week, Monday to Friday, 8 hours a day. Schedule based on the support needs of individuals in the program, including evenings, weekends and public holidays. As the program operates 7 days a week, being on-call for 1 week every 4-6 weeks is required to support the tenants and each other on the team. The on-call rotation consists of carrying a work cell phone 24/7 for that one week, responding to urgent matters.
**WHAT YOU WILL BE DOING**:
Directly Supporting Tenants
- Skilled in building therapeutic relationships with tenants
- Trauma-informed approach promoting hope and trust while maintaining appropriate professional relationships
- Program screening, intake, substance use assessments, completion, and referrals
- Work individually with tenants to identify and achieve substance related goals as it relates to their individual support needs and recovery
- Work collaboratively with internal and external partners
- Support tenants in issues related to maintaining their health and housing
- Assess complex situations and make appropriate decisions independently and as part of a team
- Confident in utilizing crisis management and intervention skills
- Develop and facilitate relevant group programming to meet the needs of the community
- Knowledge of community resources and provincial addiction treatment service

Support, Direction, and Commitment to Interdisciplinary Team
- Collaborating with staff across disciplines to assess and respond to program needs related to substance use and addiction, including facilitating bi-weekly “Prime Time” case management meetings and overseeing quarterly TIMES assessments and Coordinated Care Plan updates.
- Providing holistic, wraparound supports such as medication administration, apartment and environmental checks (including pest management protocols), community events, and life skills development.
- Partnering with tenants to develop and implement individualized care plans that support health self-management, connection to community and healthcare services, and navigation of social systems.
- Sharing up-to-date research, community resources, and best practices in addiction support, including facilitating tenant workshops and staff capacity-building.
- Supporting and orienting staff, students, and volunteers—including collaboration with the Volunteer Coordinator—and maintaining volunteer records as required.
- Promoting the values, vision, and mission of Indwell through positive communication, collaborative decision-making, and active participation in team and agency meetings.
- Ensuring safe and well-maintained environments by liaising with Facilities and reporting maintenance concerns promptly.
- Adhering to all applicable policies, procedures, and regulatory requirements.

Assessment and Documentation
- Participate in Intake process and assessments
- Lead the development of coordinated care planning with internal and external supports
- Complete required documentation in a thorough and timely manner with a focus on maintaining and protecting tenant health records
- Perform administrative tasks as assigned, such as rent collections and financial records
