Position Summary

We are seeking a compassionate and skilled Social Worker to join our multi-disciplinary team supporting paediatric patients living with Sickle Cell Disease and their families. Please note that the clinic day is on Mondays - this may change based on operational need with notice.

In this role, you will:

  • Provide counselling and emotional support to paediatric patients and their families during clinic visits and through individualized 1:1 sessions.
  • Conduct psychosocial assessments, gather case histories, and deliver short-term therapeutic interventions tailored to each patient's needs.
  • Develop and implement goal-oriented social work treatment plans and interventions.
  • Offer education and support to address psychosocial, emotional, and financial challenges faced by patients and their families.
  • Take responsibility and accountability for clinical decisions and patient management within the scope of practice as defined by the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Contribute to program planning, evaluation, and the development of resources and educational materials for patients and families.
  • Foster partnerships and community linkages to enhance patient support.
  • Participate in in-service programs, best practice initiatives, and hospital committees as required.

Key Qualifications

  • Master of Social Work (MSW) with experience supporting paediatric patients and/or individuals with Sickle Cell Disease (or similar chronic conditions).
  • Current membership in the Ontario Association of Social Workers (OASW) and registration with OCSWSSW.
  • Clinical experience in a hospital setting, with a preference for outpatient care experience.
  • Strong knowledge of community resources and the ability to connect patients with long-term social work or mental health support services.
  • Understanding of the physical, emotional, and psychosocial challenges faced by patients and families living with chronic diseases, including Sickle Cell Disease, diabetes, and other co-morbidities.
  • Excellent interpersonal, communication, and organizational skills, with the ability to build trust and rapport with patients, families, and team members.
  • A collaborative team player with strong critical thinking and problem-solving abilities.
  • Proficiency in electronic charting and documentation systems
