Job Summary:
**Responsibilities**:

- Conduct comprehensive assessments of clients' social, emotional, and psychological needs
- Develop individualized care plans based on assessment findings
- Provide counseling and support to individuals and families facing various challenges
- Connect clients with community resources, such as housing assistance, healthcare services, and financial aid programs
- Collaborate with other healthcare professionals to ensure coordinated care for clients
- Monitor clients' progress and make necessary adjustments to care plans as needed
- Advocate for clients' rights and ensure they receive appropriate services
- Maintain accurate and up-to-date documentation of client interactions and progress

**Experience**:

- Bachelor's or Master's degree in Social Work or related field
- Valid state licensure or certification as a Social Worker (if required)
- Proven experience in providing social work services, preferably in a healthcare or community setting
- Knowledge of social work principles, practices, and ethics
- Strong assessment and problem-solving skills
- Excellent communication and interpersonal skills
- Ability to work independently as well as part of a team

**Skills**:

- Supervising: Ability to provide guidance and oversight to other social work staff members
